Re: New member + split night study results

I have not gotten my results back from my most recent study on 2/4. I have uploaded the orriginal study I had on 1/28 however. I hope to hear back from the study on 2/4 soon. In the meantime feel free to comment on these results.

which machine model exactly do you have?
I have been wondering many times if I should get a PR ST machine instead of an ASV, (with which I have problems), so your answers would be very helpful to me.
Do you do your settings yourself, going into clinical mode on the machine?
do you have a link to the provider manual for this machine?
what are the settings you can set, can you give the list, with the ranges it allows?
I have been told that the PR ST machine can deliver breaths of a set length, i.e. the inspiration time can be set as a single, always attained value. Is that true, and is it true for regular breaths, not just machine-triggered breaths?
the OP did mention he was told he would be titrated for a BiPAP S/T machine, for centrals, (though on the Respironics site, that machine is labeled for COPD), so these answers could be meaningful to him also.

Did I miss it or where is the respiratory rate indicated?
and where is the backup rate that was used indicated, in the report? (Sorry if I missed it on the graphs, they are hard to see)
First, your normal, usual respiratory rate must be determined, then a backup rate should be applied at -2 from the normal rate.
So, if your normal rate was 12, your backup rate, when set on manual instead of auto, would be 10, etc.
The indication from the report, to use 12bpm in your titration, would have been based on estimating that your normal rate is 14bpm.
Although, in an ASV type of titration study, one portion of the time the RT would anyway want to use the auto rate for the backup, at least to document it.

I am the OP. Those are the results of the "split study" I had on 1/28. I went back for titration on 2/4. They had me on bipap st with a back up rate of 12. I have no machine yet, as I have yet to receive the results of my titration. As far as the settings, I'm really new to this, having only learned on 1/30 that I not only have obstuctive apnea, which I had figured, but also central apnea, I'm really just now learning. I'll certainly post more of my results once I get them, but I'm sorry, as far as your questions go I don't have any answers for you. Hope you find what works for you though.

I misread your posts. I've talked to madalot, the machine she uses is a little unique, same with her back up rate. She also has an unusual situation, which I don't feel is really mine to tell.

The chart portion of my tests make no sense to me, as I'm not trained on how to read them, and haven't done any research on those yet. For the question on back up rate.. I don't think they used it on this test. I don't think they titrated with the backup rate until the test I just had on 2/4... so that wouldn't be in the graph.

Scheduler called. I have to go for another study. Not sure when. This time they want to do asv. I've revived pretty sound guidance from folks on this site, that have laid down the path to my treatment to a tee. Thanks to everyone here. Again.
, this remains the best resource I've found for sleep apnea and treatment. Thanks for the good community.

I just now looked at your sleep study.
Yep with that many centrals and the trial on the bipap didn't help...ASV is in your future.
Good luck.
Sounds like Madalot was able to prepare you for this eventuality.
You will do fine. It will all be worth it in the end.

Yes. Madalot helped, as well as bleepingbeauty by chat. There was another in chat that was helpful as well. Im sure it will be a shock to my scheduler as I actually know what to expect. Sleep tech swarmed kind of confused that I had good questions last time and kept asking the difference between st and asv. Im glad that I happened to pick a study center that does asv... as I understand alot can't/wont do them.

Well, I have another sleep study scheduled for tonight. My sleep lab is trying to get me in as quick as possible and someone canceled, so I am in for tonight. This one is for an ASV test. This is good, because I have heard multiple people saying this is what they think I need... and have listed my path to ASV pretty accurately. My only concern is that Bipap ASV seems to be the last step, and what do we do if this does not work? I know alot of people have said that ASV is what works for Centrals or mixed apnea, but its just hard to feel good until I know that this is going to work.

Wish me luck

That post is me.

Also, what can I expect from an ASV study? Will this feel different from ST? Meaning will this be instantly more comfortable? Or is this kind of thing that still takes getting used to. Is this therapy possible to fail like ST? Or is this the one you go on automatically if ST doesn't help. If the last statement is true, does this therapy get better over time the longer the machine gets used to your breathing patterns?

That's what I hear from ASV users like JohnBFisher. It can take a few months for you to get used to what the machine is trying to do for you and with you.

Okay. So it sounds like I have to adjust to the machine... not the other way around. Sleep tech said they've never seen asv fail for a patient when everything else does. So thats comforting. I have my new report... and will list tomorrow night. Reading this makes asv make sense. I responded we'll to multiple different pressures depending on the stage of sleep. Somehow this machine cam read that and adjust... hope I feel rested tomorrow am. Wish me luck! Sleep well folks.

So when unhooking me the tech said I would get used to this treatment once I have my own machine... not sure how the test went... but asv must have helped enough to where it looks like this is where im heading. I woke up a couple of times during the study and had difficulty falling back asleep, thought this might have meant I was having problems with the therapy... but maybe it doesn't. Is this normal for first time asv? I did the same with st... so not sure how that ties in with this therapies effectiveness.

You will most likely have an adjustment period, perhaps lengthy. Don't get discouraged. Being on an ASV is more involved than cpap or bipap and tends to take more time to acclimate. It will be worth it.

There are several forum members using ASV that can (and will) help you if/when you need it.

I have no doubt its worth it. Im just concerned that the test might show its not worth it. Guess ill have to wait it out.

Thanks for the reply and encouragement.

I understand your concern, but I seriously doubt that's the case. The fact that they brought you back to use an ASV would indicate you need it. But we kind of figured that anyway...
