Well, night one on the new machine wasn't much different than all the rest except now the events are Hypopneas

Is the S9 VPAP ST one of the models that don't even flag centrals? I am pretty sure the Adapt doesn't flag centrals.
Some discussion in the past about the S9 machines used to treat centrals but don't choose to flag central events. I don't remember the whole discussion.

Well..... it's tough to get a definitive answer, but looking at this page

at the bottom where it talks about Data, it doesn't give a CAI which makes me think it doesn't do centrals.

Which means nothing really changed on night 1 of my new machine.

If the events are central, then this is exactly what should be expected.

In fixed bilevel, spontaneous breathing is enhanced by the pressure support of the machine, but in the absence of effort (central apnea), timed breaths are composed solely of pressure support. Consequently, these breaths will be smaller, so now you have hypopneas instead of apneas.

This shortcoming would be overcome by ASV technology, which operates as a "bi-level bilevel". Baseline pressure support occurs during spontaneous breathing, but PS rapidly increases during central apnea to (nearly) match the spontaneous breaths.

If the events are obstructive, the current approach is incorrect. Attempting to drive pressure through obstruction is, at best, inefficient. The airway must be opened first, then waveform optimized with pressure support (IPAP).

It would appear that one needs Enhanced AutoSet™ algorithm (with FOT) if one is going to look for centrals.

IMV, they are simply a part of the continuum than one sees during any group encounter, likened to commercials on TV, the band striking up a tune between plays during a football game (if you're in attendance) or the commentators supplying fill (if it's broadcast), awkward silence when nobody has anything to say, etc.

And actually, when you think about it, the people who P&M about "jacking" are simply part of the continuum as well.

Yet the irony remains, clinically proven again and again: A Thread About Nothing will put up a thousand views/24 hours because It's So Terrible That I Can't Bear To Watch!! (??????)

Well, this is interesting. Day One on new machine was a 14.81 with with my normal 5 AM cluster. Day 2 was an 8.34 with early morning clusters. And day 3 was a 0.0. (needless to say....no cluster).

7 hours, 16 minutes, no events.

Slept a little better maybe this second night?

Not so well that I saw this coming. Hope this is the new normal. Can't wait for tonight.

Well at least you have some encouraging reports to see now.
You know there is some truth to the "give it time" thing.
Let's cross our fingers that there will be more like last night for you.

Thanks for the analysis Dave. Last night was not a shutout. It was a 4.87, but I have an asterisk. The wife and dogs woke me at 5:21 AM and that's before I have my cluster. The weekend will give a better check.

Can you get the wife (or the dogs) to note your body position wherever possible (and obviously, the cluster-prone area(s) would be key).

The typically poor female sleep architecture could be a plus here.
