No ramp.

I think it should never be automatic, and newbies should be encouraged to only use it when they're starting.

Actually, I'd rather see those who need to be weaned onto full pressure CPAP to start therapy at a lower pressure and then increase it a little every night until they get to a good therapeutic pressure.

NO!!
Original DME "guru" (NOT!!!) set my machine up with a 15minute ramp - felt like I was sucking air through thick fabric! For me it was torture.

My respiratory therapist set me up with a 45-minute, pressure of 4, ramp. I spent the first week with that "struggling to suck in air" feeling, then started tweaking settings. At the moment, three weeks into CPAP therapy, I'm working with a 20-minute ramp, starting at 6. Now I breathe nice and easily as I'm going to sleep and it's excellent. If I ever get the hang of sleeping all the way through the night, I plan to experiment with dialing the ramp to shorter time periods or skipping it altogether. I fall asleep fast - or at least I used to, and I hope one day I will again, when I'm thoroughly adjusted to the little alien face-hugger mask.
