There are hundreds of underlying ailments that a CPAP would not cure. Many of these ailments would fragment your sleep in spite of being on CPAP, "optimally". In such cases the AHI would be meaningless. For example, you could be titrated for CPAP and later develop a CSA with Periodic Breathing and in this case you would expect an AHI of 47 and a CAI of 17 as GOOD, while staying on plain CPAP. This is b/c you probably have a Central Sleep Apnea Syndrome. On the other hand, an OSA only patient could be happy with an AHI of 5 and CAI of 1.

AHI is the first thing to look at when your sleep quality suddenly changes.
OSA is an ailment like any other. AHI helps us see how well this specific ailment is being treated by CPAP.
