He doesn't disregard several classes of interfaces. He merely states that in his opinion, the single class of full face masks is less effective at treating OSA and UARS. I believe he is right. However, if you are titrated with one in a sleep lab, it is likely more effective than if you are titrated with a nasal-only and THEN use full-face. I personally believe that full face by it's very nature requires a higher pressure. But that is my personal opinion only and not based on clinical data.

That much I agree with, I have had to use higher pressure than my titration rate to achieve sub-3.0 AHI rates with the FFM. However, I couldn't get below 5 with the nasal at all, due to the mouth leaks =(

This is a serious problem that the industry really needs to think outside the box to solve. There is no easy solution for this problem. Going back to the small jaw/mouth thing--it is precisely for this reason that I personally CANNOT WEAR a full face mask. I tried every brand including hybrid styles and Hans Rudolph. My small, "weak" chin would not allow a seal with those masks. It's a cruel joke that a whole population of CPAP users (who are users because of the small jaw) cannot use interfaces meant to help them because of said jaw. I've contacted respironics and resumed regarding this several times over the years, but never got replies. It's really ridiculous!

Indeed, I could not agree more. My hope is that as more people are diagnosed, the industry begins to see the profit in addressing issues like this. Better compliance means more ongoing revenue for them.
