I have a RemStar BiPAP/autoSV machine with Encore Smart Card and the attached humidifier, and a Respironics Comfort Gel mask. Because I have both types of apnea, I have to use the full mask that covers the nose and mouth. I also have bone on bone knees and hips, which adds to my sleeping misery.

I started with these settings for the first 6-8 months: Max IPAP-29; Min IPAP-13; EPAP-12; and Rate 12. I could not keep the mask on much less sleep when the higher pressure kicked in, and would take it off unconsciously. The doc lowered the max to 18. I still have a little problem with leakage at about 3-4 am. I end up taking the mask off so I can sleep. With or without the machine, I wake up about every 2-3 hours. I'm probably going to have to have another sleep study done as I am not feeling refreshed in the morning.

I have two questions:

1. What is the "software" I have been reading about in the posts? Is this something I can load into my computer at home to monitor things? (I am on a Medicare HMO and they do not splurge for any "extras" if you know what I mean)

2. How do I change the settings on the machine? I have read of numerous people "tinkering" to try to get the best possible solution.

Any information would be much appreciated.

Singnchef...I will try answer your questions.First as was stated you need a card reader and the software for your computer.You can find them at differant sites but I think $150.00 should cover them.

Secound.You state you need a FULL FACE mask because of your pressures.Full face have more area to keep sealed up.I myself find the Resmes Activa LT my best choice.I have tryed many and always come back to the LT.My presurres are..EPAP....14 Min IPAP.....15 Max IPAP....30

I hear you on the pain status.I have had both hips replaced,developed spinal stinoous inbetween them now and as I write this am waiting for the right knee replacement.Don't know when that will happen since I am working with 2 blood clots..

It is NOT a wise idea to change your pressure settings when you are on a device as sophisticated as a bi-level SV. Changing therapy settings on a straight CPAP or APAP is not a matter of rocket science and for the most part for most of us causes no real harm and may even cause improvement. But as you get in to more sophisticated therapy devices few lay persons have the knowledge and understanding of how one setting change affects another, how one option change affects another. In addition, doctors don't script and insurances don't pay for these more sophisticated devices unless the patient has a more complicated sleep disorder(s) than the average patient.

I also have both types of apneas. I could never find a full face mask that fit without leaking (my head is literally shaped like a bowling ball). I have found that the Hybrid works great for me. It has the nasal pillows and the mouth cover. Plus a little chin cup that helps keep my mouth closed.
