I downloaded the form and will use it if the letter from my Dr. proves to be ineffective. I hope it does not get this point. I will keep everyone posted on the result of this travestiy.

Greetings fellow hoser. I finally was able to find a DME that works with tricare, the co. name is Optigen. They were able to get approval on my ASV, if anyone is interested thier web site is thier name optigen dot net. I know we cannot post web pages here, but they work great with tricare and military personel. The RT will be coming to my house with the machine and some mask for me to try on. I will keep you posted on how it went. Thank you all for your support, it has been a long time coming.

Hurray! Glad to hear the good news. Hope you enjoy your unit. I understand how frustrating it can be to have the therapy you need be available, but somehow just beyond your reach. Here's hoping you will soon be sleeping peacefully!

Hello fellow hosers, I finally got my ASV, last night was my first night. I wish I can say it was great, but I don't know how it happened when I woke up the mask was not on my face. Somehow it came off during the night, I did some investigating and the best conclusion I can come up with is that the top strap of the mask( Mirage Quattro) became unhooked. For some ungodly reasone, we know why, the bottom line they took the snaps off and instead put a hook only an idiote would thinke would work. Probably someone who does not have Apneas decided to implement. Has this happened to any one else? Tonight will be my second night, I hope it will go better, cross your fingers. Other than that I expect this machine to be glory sent. I will keep you guts posted.

sounds like the sleep lab/doctor did everything right! In order for any insurance to pay for the asv, we had to "prove" that CPAP and Bilevel did not work. As long as we porved this, then the insurance would pay. We did hundreds of military patients where I worked and none of them were denied.
