As I read through the thread ... there's one area I haven't seen mentioned that you might want to explore.
My husband passed out a few times during a week a while back. He felt fine otherwise, so we just kind of ignored the incidents.
Then, while in the hospital for evaluation for a possible heart attack (where the patients are electronically monitored 24/7) ... the nurse ran into the room asking if he was OK. His heart had stopped for almost 6 seconds! If he hadn't been lying down, he would have passed out again.
It seems that his heart had been "misfiring" on occasion, causing him to pass out. Had he not been wired to a monitor at the exact time it happen, the hospital would not have known that his heart had stopped or what had caused his heart to stop.
He'd worn a portable EKG a couple times in the past and they never showed anything that would concern the cardiologist. It's all in the timing of the incident and the data to back it up.
His problem (and coincidentally a neighbor who also passed out and drove into a snowbank last Winter) was resolved with a pacemaker.
I'm not saying that you are having the same kind of problem ... I'm suggesting that (since it hadn't been mentioned) it might be an area for your cardiologist to look into.
