RX for cpap or apap

I am a little confused about the expiration date mentioned. I thought that RXs simply expired once they were beyond a certain date. also and as important, I was told by my insurance company that a letter entitled " a letter of medical necessity and duration" was required from the doc before they would consider paying for anything. I was told it was not a big deal but none the less they needed it. Since my sleep doc is an idiot ( I am being unkind to idiots) I need to get everything in order before I get him involved. I guess there is a chance his office staff knows what they are doing. Does anyone have the typical verbiage for duration? Since I want an apap for the ability to monitor how I am doing (a pressure of 12 is fine, don't use ramp) do I just have the doc write an RX without a pressure?
