Mr. Smiley wrote:Well I got my numbers first thing this morning. And a question. My numbers are Leak: 0.08 L/s AHI 8.4 AI 1.0 and HI 7.4 Is this good?
I think those numbers look
very good! Especially since I'd mentally cut the "HI" in half if you're going to compare your overall AHI to the AHI people using Respironics machines talk about. I don't know this for a fact, but I believe ResMed
S8 machines tend to report as "hypopnea" a greater number of flow limited events than a Respironics machine does. It seems to me that some of ResMed's
S8 hypopneas would be called just a "flow limitation" in a Respironics machine. Anyway, that
seems to be the case when I switch between brands of machines, and get equally good treatment from either but see double or triple the "HI" number reported by the ResMed
S8 machine.
So, cutting the ResMed
S8 HI in half and using that "half HI number" to add to the AI, would give you an overall AHI of 4.7 instead of 8.4. Nice.
If it were me, after a couple of weeks, I'd try a pressure of 13 or 14 to see if that would reduce the AHI even more (still always mentally cutting the HI in half and re-adding that to the AI to get the "AHI.")

Except for the added setting for "EPR Inhale" in the Elite II, the menu is probably essentially the same as for the older Elite. Here are a couple of old topics that might help you figure out your machine's menus:

Mr. Smiley wrote:From my take on this, i only have the EPR during the 25 min ramp to my full pressure.
Right -- your EPR is set to operate only during "ramp." I'd raise the ramp start pressure. Starting ramp pressure at only 4.0 cm is awfully low, imho. Can feel stifling trying to breathe through a mask at that low pressure. If I were going to use ramp (I don't) I'd want the pressure to start at least at 5 or 6 cm.
Mr. Smiley wrote: Id like to have it all night, as when i woke up last night it was more difficult to go back to sleep with out taking a sleep aid.
ozij had a great suggestion about just reaching over and hitting ramp again. I'd still adjust the starting ramp pressure up higher than 4.0 cm, unless you like that and want it down that low. But if you want EPR on all night (I would want that), EPR can be changed to "full time" in the clinical setup menu.
Mr. Smiley wrote: Please correct me if im wrong, I was told that the Dr. ordered a level 3 on the M-Series plus machine and that it is a level 1 on the ResMed, the highest setting. I read in the user manual that level 3 has a better relief then level 1?
You're right. The manual you looked at is correct. Whoever told you that EPR 1 (a one cm drop in pressure when you exhale) gives the most EPR relief, or is the same as C-Flex 3, was wrong.
C-flex 3 gives the most pressure reduction C-flex can give for exhaling, but not in exact cm's of pressure reduction.
EPR 3 gives the most pressure relief EPR can give for exhaling, an exact 3 cm drop in pressure during exhalation.
I'm not a doctor or anything in the health care field, but if I were going to turn on EPR full time (instead of just during "ramp"), I'd raise my prescribed pressure a cm or two. You've probably already read my thoughts about machine pressure setting when using EPR, Mr. Smiley:

My edit: Added "S8" to all my references to ResMed machines in this old post, since the definition of hypopnea was changed by ResMed in the later S9 series machines.
