I think it is important that doctors take it seriously when someone is on antidepressants and still complaining of extreme fatigue.
I once said to my psych that I thought my depression symptoms seemed to revolve around sleep and asked for a sleep study. He said no.
It would be months later before my GP made the suggestion that I get one. Based on the size of my tonsils. A total fluke. But one I'm glad happened or I'd still be suffering and my son would have been an only child with a mother with tons of health problems. Now he is going to have a baby brother some time in the next month and I'm healthier now than I have been in years.
I haven't seen the questionaire they mentioned but have heard good things about it. Some simple questions would have done the trick for me years ago if it were widely used.
