Nothing you can do to increase EEP without getting the enhanced VPAP Adapt SV or a Respironics BiPAP Auto SV.

The snoring is BAD and getting worse! Sounded like a freight train this morning.

I have all the info...

I've got everything ready. Can someone to tell me how I should post it all or which info is the most important?

Thanks!

Casi

I have the data - a good sample - have had a prelim look & will study further then get back. I see the Reslink was used on some nights - that data seems ok. No serious desats from 1st glance.

He forgets to use the sat monitor all the time. He told me it was my fault for not reminding him...typical. lol.

I saw there was one night where his sats were in the 70's for 19 sec, the 80's for 19 sec. Thats scary to me.

Sorry it took so long to get there. I guess it was a pretty big file. He actually goes back to doc 1 at 11:30 am today. Should be interesting.

How do I post the info here (and which info) for everyone to see?

Done

I'll look closer at the SpO2 when I get the chance. The biggest problem I see is mask leaks - I tend to have similar with the Vpap SV.

I can host them for you

We talked about the leak/snore (I say its snore, hubby says its a leak) issue with Doc 1 today. She thinks we are probably both right - some snoring and some leaking. She has a theory and wants him to try some new nasal mask.

Thanks for all your help DSM!

A new nasal mask on an Adapt SV? I don't think so.

But hey, how do you get the 'Detailed" Report from the ResCan SW? All I could get was the 'Summary' Data/Report.

Yeah, its a nasal mask. She says it will work with the SV (thats her theory).

Lol, DSM. Casie isn't good at anything that involves computers.

Casi,

I have trouble with ResScan & getting reports, it surprised me how quickly you followed the directions - I wasn't expecting a reply for 24hrs but you got back in mins

Re the report - here are my impressions

OVERALL - the machine seems to be doing ok but there are periods where hubby's sleep gets broken. I see several instances of 30min to 70mins gaps in the night where no data is recorded. Makes me wonder if hubby has some factor causing sleep disturbance outside of pure respiratory causes.

When he does get good sleep, the data is very good & his Tidal Volume is healthy & consistent. The nights with the SpO2 do show disturbed sleep & I am wondering if wearing the probe actually causes this ?

TRENDS DATA
==========

PEAK INSP PRESSURE: - no issues
EEP: - no issues
PRESSURE: - seems a tad lower than I would have liked to see
LEAK: - Lots of leaks - big ones & fairly common night to night.
TARGET MV: - ok
TIDAL VOL: - ok for median & 95th % but the max does go questionably high
MINUTE VENTILATION: - same as above
RESP RATE: - shows bursts of hyper-ventilation up to very rapid rates - needs looking at (see 2nd & 4th July)

Below I looked at the data for the 4th to the 14th. Pls do get back with any comments from your RT re this data (helps me better understand the trends).

DSM

DAY-TO-DAY
=========
14th
PR = good
LEAK: = ok
MV = good
RR - good
TV = good

13th
PR = good
LEAK: = few rough spots
MV = ok
RR - good
TV = ok - some arousals ?

12th - 2 distinct breaks in the night
PR = ok
LEAK: = moderate
MV = good
RR - good except for the breaks causing disruption
TV = ok but there are a few quick drops

11th - a 1kour 10min break in the early part of sleep ?
PR = ok
LEAK: = moderate
MV = good
RR - good (except after the breaks)
TV = ok but some highs

10th
PR = Very good (except for the 1 hour break)
LEAK: = moderate
MV = ok
RR - ok
TV = ok (some peaks showing)

9th
PR = ok (seems higher than other nights)
LEAK: = ok
MV = good
RR - ok but showing some peaks
TV = ok
SpO2 = good
PULSE: good

8th
PR = ok
LEAK: = ok
MV = good
RR - good
TV = ok
SpO2 = shows a disturbed night (is the probe interfering with hubby's comfort ?)
PULSE: ok

7th
PR = ok
LEAK: = problems
MV = good
RR - good
TV = ok
SpO2 = good
PULSE = ok

6th
PR = ok (some fluctuation)
LEAK: = ok
MV = ok

DSM - I doubt the sat monitor bothers him that much. U could set a bomb off next to him and he would sleep through it.

His mask has a small tear that I noticed this morning. I called and ordered him a new one. It will be here tomorrow. I don't know if its big enough to cause the leaks or not. I know doc 1 wanted him to try a diff mask but they haven't gotten the order yet. The DME said they would just send it to us when they get it.

I really don't understand what all the info means. I understand what each setting is for but more than that and I am lost. I guess my #1 question is ... do yall think he belongs on this machine? I know there were questions about cpap vs bipap at the last titration.

The reslink actually has all the data from March-July on the reports. I guess its not as detailed but maybe it would give more info on his trends.

Casi,

I am not sure this is the best machine for him but would like to hear from any of the others SAG, SWS etc: as to any thoughts they may have.

He doesn't seem to need the extra pressure support offered by this particular machine. His pressure doesn't vary that greatly.

The periods of rapid breathing are something that needs an answer to. Is it because of this machine or is there some other issue ?.

But IIRC, when he was on the Vpap III he had much worse nights than now. If I could ask for any data I wanted I would ask for a few nights data on a BipapSV & from that plus what we already have, believe I could take a stab at what is best.

AVAPS!

Average Volume Assured Pressure Support or AVAPS is a new method of delivering a consistent tidal volume through a unique algorithm which evaluates the patient's tidal volume over several breaths and then calculates the amount of pressure needed to reach the ideal tidal volume. The machine then gradually raises or lowers the IPAP pressure to achieve the proper pressure support.

Since the pressure changes smoothly, it does not compromise patient comfort or safety. Leaks are taken into account so that AVAPS delivers a true averaged patient tidal volume.

For maximum flexibility this machine may be run in the following modes: CPAP, Spontaneous (S), Spontaneous/Timed (S/T), Timed (T), and Pressure Control (PC).

The BiPAP AVAPS may benefit COPD and neuromuscular patients, obesity hypoventilation patients, as well as restrictive patients since it delivers the comfort and leak compensation of a pressure mode and the safety of a guaranteed volume. Also, because the algorithm compensates for changes in body position and maintains ventilation the algorithm supplies the right pressure at the right moment.

Advantages
Adjusts to changing variables and needs
Improves ventilation effectiveness and comfort
Optimizes safety by delivering an average tidal volume
