Grrrr. I wrote a summary of the article and thought I succeeded in posting it, but ... no. I now only want to add a few thoughts to the thread that has developed.

Although flow limitation is one of the variable used in comparing the 10 machines, there's no information about EPR (or similar) settings.

The main point of the article is to provide a "proof of concept" for a simulation of how machines respond to breathing patterns thought to be typical of various sub-populations of APAP users.

ResMed comes off well using AHI and FLs as the key indicators. The "For Her" version uses lower pressure to get similar results to the ones recorded for the regular Airsense 10 Autoset. (The selected profile for the simulation discussed in the article is that of a woman, for what that's worth.) PR comes off badly. F&P does well for AHI but not for FLs.

There were some interesting take-aways.

1. Some providers have provided machines with the settings at 4-20. The study and Nick point out that in some cases having the 20 top end is detrimental, because the machines may respond to an event with more pressure and longer than it's actually needed.
Creepy "uncle" dicko knows so very little.

If the machine has the MINIMUM pressure set properly, then it won't *NEED* to ever come close to the max pressure.

All you do by lowering the max pressure, for MOST people, is prevent the machine from being able to respond if you need more pressure for some reason, like REM after rolling onto your back.

Warning folks....don't go trashing the thread with comments about comments or people you don't happen to agree with.
It's fast turning into a cesspool of crap talk and that's not needed or appreciated.

I can still lock threads and/or remove the thread or posts.

These machines won't go higher with the pressure unless it thinks it has a valid reason to go higher. Is it actually "needed"??? Well that depends on the why it goes higher but the only time I can think of where it might not be needed is because it is responding to flow limitations that are nasal in origin like bad allergies or a cold or some other reason for nasal congestion.
More pressure won't help FLs that come from swollen nasal mucosa. Gotta go about that fight in a more traditional manner.

As for more pressure causing leaks.........well....people use pressures every day of the week exceeding 20 cm because they need that much pressure and leaks are probably better controlled than my leaks with my pressures half that amount.
Proper mask fitting is needed to be able to deal with high pressures. People get it done every day and leaks or pressure don't wake people up. Hell when I was using the ASV and seeing pressures upwards of 24 cm when PS jumped in for a central I slept right through it. No leaks and no wake ups from the pressure and I slept right through it and never knew it happened until I saw a detailed report.

Play nice folks. Getting shitty with people isn't playing nice and isn't needed and guaranteed to piss the moderator off.

Sometimes restricting the pressure is needed but it needs to be done on a case by case basis and depending on what is happening. Again...just because a machine can go higher doesn't mean it will and it usually has a good reason when it does go higher. Is that reason a "good" reason???? That's what needs to be evaluated on a case by case basis.

If you all continue with the shit talk about each other that puts this thread totally in the cesspool toilet and I will stop it.
