Re: Still Tired after 1 year + Analysis
I had forgotten to specify, I changed the fixed pressure from 7 to 6 from the 2nd day.
I also post last night.
What do you think of these new scores? Is it useful to change new settings?

It's useful only if it helps something of course. How do you feel today?

Re: Still Tired after 1 year + Analysis
one of the things that will help you in your journey is to not think of the numbers you get as a "score". yes, generally speaking, the lower the ahi, the better treated your sleep apnea is.
however, as julie points out, it's most important to let us know how you feel. if you feel fine with these numbers, then viola! there you are. no need for anything more to be done.
if you still feel crummy, give it a few nights at these numbers, then report back.
good luck!

Re: Still Tired after 1 year + Analysis
Thank you for your help.
I can't say if it's totally fixed because I have a busy work week so I was still tired and had a very bad night Tuesday-Wednesday and Thursday-Friday.
For yesterday, I was rather tired despite the 8-9 hours of sleep. I yawned many times during the day, I even took a 20 min nap.
Today I don't feel too bad.
The coming week will allow me to see if there is a real change. We will see if the fall of leak rate really has an influence or not. I will keep you posted in the days to come.

Re: Still Tired after 1 year + Analysis
Last week was complicated,
I've been quite tired with naps on Monday, Wednesday and Friday for instance.
I don't feel like the change in pressure has changed my condition much. I always want to take naps during the day. I have also moments of hesitation, the impression of being completely elsewhere. However, I have the impression of dreaming a little more and above all of remembering my dreams more precisely.
Tried EPR on level 1 for 2-3 days then turned it off.
I put some captures of OSCAR. I don't really know what to do anymore.

Would you try e.g. 7 or 7.5 for a night or two to see how things go - 6 is quite low after all.

Listen to what all these people are telling you:
They correspond to lowered flow in the flow chart and to the presence of flow limitations in the flow limitations chart.
This happened when you used the FFM as well - drops in flow, accompanied by flow limitations and leaks
you have to find a higher pressure that will keep those drops in flow from happening.
ElusiveSleep wrote: ↑Mon Mar 06, 2023 12:18 pmYour OSCAR results look very similar to mine. The large leaks are mainly due to mouth opening and they coincide with increased flow limitations. As the airflow becomes restricted, the mouth opens involuntarily to bypass the constriction of the area in the throat at tongue level and above.
Those jagged, toothlike leaks in your "constant pressure charts" are mouth leaks.luciferin wrote: ↑Mon Mar 06, 2023 1:13 pmYou've got to deal with those leaks, my friend. Anything could be hiding in there. I had very similar looking leaks with nasal pillows. Turns out I have an upper airway obstruction or floppy soft palate that restricts nasal flow and can force my mouth open. And it happens to me roughly every 90 minutes, right when I start REM sleep. Sometimes I'd be so exhausted that I'd sleep through it with my mouth open, more often it would slightly wake me and keep me from REM.

In your case, you have to look at what happens on all the charts together. Your sleep - its quality - is described by all of them together. You wouldn't describe a symphony of music by the number of times you hear drum beats, nor would you describe a rock group's music by focusing on guitar plucks alone. There's much information in your charts that isn't expressed by a "score", and you have to try to understand it in order to get the kind of sleep that will keep you from being so tired.

Re: Still Tired after 1 year + Analysis
Thank you for your answers.
I thought the leakage problem was solved since I had gone from sometimes 15-10% of Large Leaks two weeks ago to 2-1% (even 0.5%) by putting constant pressure.
I will try 7-7.5 constant pressure from tonight. I'll keep you informed.

Re: Still Tired after 1 year + Analysis
I feel very nauseous. I had a very difficult night with the change in pressure, there are even more leaks. I am attaching a screenshot of OSCAR.you have to find a higher pressure that will keep those drops in flow from happening.
Should I increase the pressure again? Or decrease it?

Re: Still Tired after 1 year + Analysis
From 6.2 to 7.4 is clearly too big a leap - and I can see how bad the night was.
When I said "to find" I should have said: "search for a higher pressure gradually."
Think back to which was your best night - subjectively - and see what a small change from there upwards will do.
What are you doing against mouth leaks? You can't get therapy when you're mouth leaking.

Re: Still Tired after 1 year + Analysis
Thank you for your answer.
I feel like the stronger the pressure, the more air I have to hold in my mouth (and then it causes leaks when I sleep, right?).
I'm having a little trouble understanding the idea.
I will try to increase the pressure slightly (above 6 therefore).
So far, I don't have any strips yet. I just do mouth strengthening exercises.

There's a valve between your nose and your mouth, which is usually closed.Jimmy-Bills wrote: ↑Thu Mar 23, 2023 10:25 amThank you for your answer.

The higher pressure forces it open.
Some people, like me, never manage to keep the air from entering their mouth and blowing out through their lips when they sleep. So far, I don't have any strips yet. I just do mouth strengthening exercises.
You will have to find out which helps you sleep better: a full face mask or the various methods we use to keep lips shut.
Zonker, for example, uses elastic headbands (called Scunchi).
I have to fully tape my mouth, or use a full face mask.
Finding the correct solution will take time.
Have you ever had your machine on automatic pressure? If so, for how long?

Re: Still Tired after 1 year + Analysis
Yes I was in automatic pressure for a year and a half. And I was always tired, muddy.
I posted some screenshots on the front page. There were a lot of large leaks.
Switching to constant pressure at 6 reduced Large Leaks from 15% to 2-3% (see 0.5%). But the fatigue is still present.

Re: Still Tired after 1 year + Analysis
So, Am I testing 6.4-6.6 for example? Or I can also test less than 6?
