Looking for Advice on Resmed Air Curve 10 BiPAP - ST-A

Hi everyone. I'm new to this group. I have done a lot of reading recrently and have been on BiPAP therapy since 2008. My sleep specialist recently upgraded my machine from a regular ResMed Air Curve 10 BiPAP to a newer ResMed Air Curve 10 ST-A unit.

I have to admit I'm very frustrated as I have been 100% compliant all these years using these machines, but still am incredibly fatigued most days and feel like I get poor sleep. I also have struggled greatly trying to get any decent mask fit w/o big leaks. I was looking at my reports with the new machine and for 7 days it has an average mask leak of 65L/hr. My wife wakes up constantly as she says she hears gurgling sounds and whistling when I'm breathing. I use a ResMed Quattro full face mask. It seems like it leaks when I open my jaw at times (from the bottom) and also I have terrible dry mouth upon waking and extremely dry eyes.

Some settings on my new machine are: EPAP: 10.0; Min PS: 10.0; Max PS: 20.0; Ti Min: 0.3s, TI Max: 2.0s,; Target VA: 5.0 L/min; Target PT Rate: 15; IVAPS Mode. A recent report says spontaneous breathing occurred 87% of the time. No Central Apneas were recorded during any sleep studies. I'm morbidly obese with pulmonary hypertension. Recent sleep report for a week showed Exp Pressure of 10.0; Vt: 800 ml; RR: 13; MV 10.8L/min, TI 1.1s; I:E: 1:3.0.

The question I've asked myself for years is am I not getting any real effect from therapy based on the mask leaks? Four times I had to have my doc pull pressure down from 21/11 to 16/8 due to not being able to handle the extreme air pressures. At every follow-up appt, I feel like I'm told new settings or a new machine will be life changing, but NEVER happens. I'm always yawning 200 times a day and can barely make it through a day. 8 hrs of sleep is unrefreshing. Doctor's office says my AHI index is well under 5 with the settings used during polysomnograms. My new machine is claiming AHI of 0 to 1 most nights, but I don't believe that for a second honestly. Makes zero sense to me.

I have struggled a bit to understand why I was moved to the ResMed ST-A machine, but I assume there was good reason as the insurance had strict requirements to quality. My doc made comments about excess CO2 retention due to obesity. All lab work always showed normal 'high range' CO2 levels on the CMP lab test (29-31) and Arterial Blood Gas testing showed normal CO2 levels recently. I understand both these indicators are not reflective of sleep levels.

Any feedback / advice would be immensely appreciated. I hope to learn a lot here.

Please remember when you're looking for information, Resmed never has, and never will make anything called a "BiPAP", that's a registered trademark of philips.

You said you had some sort of Aircurve (which means it's one of 5 types of bilevel machine) but you didn't say *which* of the machines you had before.

Thanks for the insight. I was not aware of the naming issue/trademark stuff. I've always been prescribed Resmed machines. I will need to look at an old machine to see if I can identify which one it was. It was silver and an Air Curve 10. I'll need to identify further labeling on it and update here. My machine was 7 years old so my physician replaced it.

Just look at the name on the front of the machine, underneath "Aircurve 10".

I uploaded information into Oscar for the first time ever. I have a ton to learn on this stuff. From initial glance, my apneas seem pretty controlled which makes me wonder if sleep is not the true reason for all the fatigue. Total average pressures seem to be mostly around 20-21 cm IPAP over 10 cm EPAP. Extremely low AHIs most days. My leak rate last night went WAY down upon moving to a new mask: F&P Vitera. My spouse said she heard no noises unlike previous nights. I always have extremely stuff muscles/joints regardless of reported results on the machine.

Being overweight and suspecting too high retention of CO2 and daytime fatigue. Highest CO2 in daytime was 32 on lab test and normal range on Arterial Blood Gas analysis. There could very well be more I don't understand that explains justification.

Those leak numbers are pretty bad and I would think they're also significant.

Not only can large leaks affect the efficacy of therapy directly, but leaks, even small ones, can also negatively affect overall sleep quality.

Any chance you are waking up because of the leaks?
Joined as robysue on 9/18/10. Forgot my password & the email I used was on a machine that has long since died & gone to computer heaven.
