New ResMed Auto

My sister and I are RRT's. My Script reads 4-20. that's it. My sleep study was Mild Sleep Apnea with Hypoxemia. Not sure what setting to use. VAuto, S or Cpap. Is the epap 4? and the Ipap 20? That seems awfully high. and no PS. Can someone help me out with that script.

To save us guessing:

1. What's an RRT? Please don't assume that everybody knows every abbreviation.

2. What machine do you have? Exact make and model. It sounds like you have some sort of bilevel but your 4 - 20 "prescription" is just the factory default settings for an autoset.

3. Please post a copy of your sleep study report and prescription so we can see what's actually what. Don't forget to erase any personal ID from the report.

That's not a prescription, that's a *COPOUT*, it's nothing but the limits the machine can be set to.

Well, unlike respirator99, we can make an educated deduction based on the question, and that is that you have either a S9 VPAP Auto (unlikely since they're previous generation, or an Aircurve 10 VAuto, (since those are the only machines that have a "VAuto" mode.

The only mode that will accommodate a pressure range is the VAuto mode.

An EPAP of 4 and IPAP of 20 would be a PS of 16.

The VAuto mode has a MinEPAP, MaxIPAP, and PS settings.

If you wanted to ape the useless script, you'd set the machine to MinEPAP 4, MaxIPAP 20, PS somewhere in 0-3, but that would be a mistake, because almost nobody is adequately treated with an EPAP of 4.

A more *realistic* starting setting would be MinEPAP 6, MaxIPAP 25, PS 3, then pull the data from the machine after one night and post it so that folks here can advise you how to adjust the machine for night 2.

Sure, wad it up and throw it in the trash, because it's worthless.. well, I guess you can hang on to it in case you need it to buy supplies down the road. It was written by someone criminally ignorant of reality.

PS, what's an RRT? (If it's Registered Respiratory Therapist, as suggested, then that's a perfect example of why we in the CPAP world tell patients to ignore things that RTs tell them, because most RTs don't know ANYTHING about CPAP therapy).

Thank you for all of your comments and Helpful advise. Let me please clarify. I have been out of the circuit for 9yrs. And i am not up on the current machines. I have not worked in the sleep lab either. I happen to come along to a Resmed S9
and i am trying to figure out settings. Thank you all.

If you don't have the clinical/provider manual for your machine go here and request a copy be emailed to you.

We have no way to know exactly what settings you are going to need but a good starting point would be

VAuto Mode
Minimum EPAP 6
PS 3 or 4 (try both and choose the one that feels the best)
Maximum IPAP 25
All the comfort features like Ti min and Ti Max, etc....set those to the factory defaults (explained in that manual) for now anyway.

See what happens and follow up with using OSCAR to further optimize treatment if needed.
