NEW USER--BIPAP MACHINE AND MASK BOUNCING

I have been a CPAP user for 7 months with limited success.

I had a sleep test and it was determined that I need a BiPap Machine.

I use a Respionics Full Face Mask which I like.

I have had the BiPap Machine for a few days but I am having difficulties.

My machine is set with a ramp up.

I am having tremendous difficulty with the air surges. It keeps me awake. I know that I need the air flow this way but they seem extreme.

Further, the mask is inflating and deflating I believe this is called a bounce.

When I loosen the ask I get leaks.

When I tighten the mask I have less bounce but the back of my head and neck hurt.

Do I need to investigate if the pressure is set to high. The therapist said I was set at a 12 with a ramp up.

I tried other masks that I have and the bounce is even worse.

Any advice would be helpful.

Can you tell us exactly which BiPAP machine you are using? There are different brands and different models, and in order to help you help yourself, we need to know what machine you are using.

Respirionics makes more than one full face mask. Again, it would help us help you if you could tell us exactly which mask you are using.

There are several things that could be going on, many of which are "normal", but irritating for a person who is either new to xPAPing or is having trouble adjusting.

But in order to help you, we need to know about your clinical settings: What mode are you using---fixed bi-level pressure or auto adjusting bi-level pressure?

What is your EPAP (or min EPAP) set to?

What is your IPAP (or max IPAP) set to?

How is the ramp set up? Beginning ramp pressure? How long is the ramp supposed to last?

Do the air surges bother you right from the start of the night? In other words, when the ramp is on, do you still feel like there are air surges? Or do they start after the ramp is off, but you are not yet asleep?

The air surges are most likely the machine transitioning from EPAP (pressure on exhale) to IPAP (pressure on inhalation). When a bi-level machine is set up in the most compatible way for your breathing, those changes from EPAP to IPAP should not really be all that noticeable. But when some of the settings on a bi-level machine are not set up quite correctly, the change from EPAP to IPAP can be quite noticeable and it can feel like the machine is out of sync with your own breathing rhythm. Sometimes folks will complain that the machine is "rushing" them to breathe. Sometimes they'll say the machine is "rushing" them to inhale before they're done exhaling. Sometimes they'll say the machine is not letting them fully inhale before switching to EPAP and that leaves them "air hungry". The fix (or fixes) depend on exactly what the problem is and what machine you are using.

If the mask is properly fit at the pressures you are using, mask bounce should not be a big issue.

But many DMEs do not teach new PAPers how to properly fit a mask. And in many DMEs, the mask is fit while you are sitting up.

The headgear needs to be loose enough for the air cushion on the mask to fully inflate. If the cushion cannot fully inflate, then you don't get a good mask seal and the result is leaks.

I would suggest trying to fit the mask while you are lying down in your preferred sleep position with the pressure at your full therapeutic settings. Facial structure changes depending on whether we're sitting upright or lying in bed. When you first turn the machine on, it may help to also gently pull the mask a bit away from your face and then let go so the mask can "settle" on your face.

You might also find using a mask liner could help with this issue.

Again, we need more details: Is that 12 the EPAP? or the IPAP? or are you still using a CPAP/APAP while waiting for your bi-level machine to be delivered to you?

And what is your starting ramp pressure?

To help us help you, we need to know more about what is going on at night when you are trying to use the machine. The easiest way to figure out whether your pressure setting is too high is by looking at what is called the detailed daily data for several nights. But that means you need to first figure out whether your machine is compatible with a program called Oscar or a website called SleepHQ. If you are not using a PR DreamStation or a Resmed AirSense or AirCurve, then chances are that your machine is not compatible with Oscar or SleepHQ. In that case, you may need to get a hold of the professional software for your machine; we can help with that if you can't use Oscar or SleepHQ with your machine.
