jolting - waking up and insomnia or not using cpap

Post by dacotto1984 » Sun Aug 28, 2022 6:51 pm

Hi im having an issue. Im not sure if this is something mental or physical.

so i was diagnosed with severe sleep apnea (obstructive sleep apnea), was having about 90 events per hour when i first did my sleep study. blood oxygen was 80% or so.

when i first did started cpap i did it for a month, i was breathing in very hard and felt like i wasnt able to breath with the mask on. breathing in hard was causing my lungs or chest to burn and hurt. so i stopped after that first month.

so after 6 months after quitting using the cpap machine, One night, i experienced a feeling where i couldn't breathe at all when i tried to sleep.. as soon as i tried to lay down, it was like i was just totally choked or my whole windpipe was not working at all. I felt tight all over my head as if my windpipe or throat and head were just shut (i know this sounds weird but its best i can describe it).

After a while i was experiencing total insomnia. i was waking up jolting all the time and just felt super tight all over my mind. when i tried to sleep it was like someone was zapping me up or something and i couldnt rest. I had to go back to sleep doctor and i started using the cpap machine again and it was very hard to start off again. I was experiencing insomnia. I couldnt sleep at all and i was just laying there not able to sleep cause i kept on jolting awake even with mask on.

mind you, i believe i was having insomnia due to some personal life experiences as well, i had my first anxiety attack and thought i was going to have a heart attack or a stroke etc. I was also going through alot of stress with work, home, cars, financing, wife and kid. So im not sure if i was having so much stress that i couldnt sleep or if i somehow developed Central Sleep Apena. (i read OSA can lead to CSA)

Anyway, before cpap and the personal events that happened in my life, i use to sleep long hours. ill sleep at midnight, wake up 3 times a night to urinate and go back to sleep each time and wake up at around 9am. but now, ill sleep at 1030pm and wake up at 3am or 4am and not be able to sleep. if i have to wake up to urinate, i am not able to go back to sleep at all, ill just be up all night. I get about 3 to 5 hours of sleep at night. and i never experienced this before. However, i am not really sleepy or tired or take naps during the day like i use to, however i am not sure if its cause the cpap therapy is working very well and my body no longer needs the 8 - 9 hours of sleep and my quality of sleep has been condensed to 4 - 5 hours or if i am experiencing insomnia and my brain is still having issues sleeping.

if i dont use the cpap machine, ill have those experience of jolting and when i use cpap i still feel tight but ill eventually sleep and breathe. my pressure averages about 13.. anyway, does this sound familiar to anyone? I feel like if my cpap were to stop working that ima die in my sleep or that the 'mental' tightness is gonna get so strong that ima end up a vegetable or brain dead. When i first did my sleep study, i slept that night just fine with 90 events an hour, no 'mental' tightness, no nothing, i think i slept better then than i do now , even though i dont feel tired during the day now.. but then again.. not sure if its cause cpap is working very well or if i am having insomnia.

anyway, sorry for long post, nobody might even read this but any help would great. thanks

welcome to the zoo!

thank you for writing in paragraphs. while what you say is a lot to take in, at least i can read it whereas so many people just type a "wall of text". so thanks for that.

what you are going through isn't all that uncommon. some folks can put on the mask, fire up the machine and all is well. can you believe that? :) while other folks like you and me have to go through quite a lot to get where we want to go.

i'll tell you what. not all of us are on the same journey. we don't even all have the same machine or mask. and this can make a difference on the advice given.

if you would be so kind as to tell us which mask and machine you have, that would be a start. if you go to your profile and fill out that, it will appear at the bottom of each of your posts, as mine does below.

but can you also tell us what pressures your machine is set up for? i suspect that your pressures (though they may have been set for you by professionals) aren't really correct for you. but need to know what you're set at would help.

finally as we go onward with your therapy(btw, this right here is your therapy thread and it would be great if stayed in it has we get you sorted out.) you'll want to get the free software, oscar. see this thread to see what that's about-

Zonker is right. The good thing about this therapy is you can get actual data to see what's going on with your treatment instead of wondering and guessing. if you have perhaps started having central events, it will show in your machine data. Sounds like data is going to be key in you optimizing your therapy and then being able to relax and lean into the treatment and sleep. If after getting settled in to using CPAP you are still experiencing symptoms you can consider if any other sleep disorders might be at play. I wouldn't chase new things until you've ruled out things most likely. Good luck.

Brain zaps are a thing. Although I don't usually recommend .com web pages, I think this one is good:

https://www.anxietycentre.com/anxiety-d ... rain-zaps/

See if any of this fits your experiences. The good news is that they tend to go away over time. Addressing anxiety and stress in your life might accelerate the process, and if you've taken antidepressants, you may want to talk with your doctor about your experience with zaps.

Zonker is right. I experienced similar waking "jolts" when I switched from Philips to Resmed last year using similar settings. Adjusting the minimum pressure up slightly (I think it was only a half cm) took care of the problem. See viewtopic.php?f=1&t=106794&hilit=cpap+basics for a few pointers on entering the clinical mode.

Hey Zonker and everyone else,
thank you very much for all the information!

I suppose my issue has to do with anxiety but i hope data from my machine can shed some light if its something else.

I have updated my machine settings in my control panel.. I couldn't find my machine with just Airsense 10 AutoSet with humidifier WITHOUT any bundle. I have a ResMed AirTouch F20 Full Face mask Large size with cushion which I bought off ebay. It leaks pretty badly sometimes. Im not sure if this mask is for this machine. The sleep therapy people gave me an AirFit F20 Medium Full Face mask but my jaw drops during sleep and doesnt cover my whole mouth so i bought this large ResMed full face mask on ebay without really knowing if its 100% for my machine.

I have downloaded OSCAR on my machine, i wish to share my information to see if there is anything i can find.

Are there any specific instructions on how i can do this so i may post it on here.

Once again thank you for that link and thank you all for sharing! I really look forward to learning so much from this community! :)

Look down in the "Rs" for ResMed AirSense 10 AutoSet....no bundle.

You are most welcome but I see you chose the AirSense 10 CPAP machine ....
and that's not the ResMed AirSense 10 AutoSet....go looking again and scroll down to the R's to find your AutoSet.
For some weird reason it got stuck down in the Rs and not up in the A's with the rest of the AirSense models.

So all my life i slept on my stomach, i never was able to sleep on my back or any other positions besides the position you see in this image https://ibb.co/sQnWw24

Im having a big issue where my cpap full face mask doesnt allow me to sleep with my face plastered to the pillow the way i like. Its driving me crazy cause all my life i slept this way and my body is not getting use to any other position.. i was wondering if there is a solution to my dilemma. Are there any pillows or types of mask for this issue.. any help would be great

You can try adding a pillow so one side is higher, a la Falcon position, to create space for the mask.

See the wiki article wiki/index.php/Sleep_Positions

There are CPAP pillows - bought or self made - that could help.

Stomach sleeping is often an intuitive unconscious choice of people who suffer from OSA since in that position there's the lowest probability of obstructing your airways.
