so if apnea is 80-100% reduction in flow, it's whatever brings it to 0 flow? How does the % reduction work. Sorry if stupid question.

The machines measure the flow of your breath, computing a running average, and a reduction in the flow will be considered an apnea, or a hypopnea. Any reduction in flow below 80% of that running average is an apnea.
The pressurized air is supposed to help keep you air flow as close to 100% as possible.

The therapy's aim is keep your airway open so that your sleep breathing flows stably, at a constant volume without too many reductions.

Here is today's Oscar graphs,a few more centrals mostly toward the end, I have chart zoomed I don't know if these are from waking up or what the breathing before and after means. Seems like a lot more leaks than usual.

You didn't zoom in enough. You have to be able to see each breath to evaluate the shape and pattern.

Is that zoomed enough ? There are a bunch in a row but I don't know which part of the breathing is most helpful to see.

This is zoomed closer 22 sec CA, and there was another CA right before it and another maybe 3 min later

this one you didn't include enough of the flow rate immediately prior (1 minute at least) to adequately evaluate it.
The breathing after the flag is obviously awake/arousal breathing but we can't see enough of the breathing prior to the flag to make a determination.

The breathing in front of the flagged event...at least a minute or so.
Often the flagged event has to be to the far right of the graph.

I know we like to cram as many flagged events as we can into one image but when we do that then it's hard to know what all started the cluster of events. I have seen one real OA (on my own reports) trigger an arousal and then several awake/arousal related event flags. It's rare that I have that happen now but occasionally I still see it happen. This is why it is important to deal with OAs....because they cause arousals. The arousal related flags really aren't important in terms of AHI because they don't count but in terms of being related to crappy sleep because of the one real asleep OA...now that crappy sleep is important. When we see a lot of arousal breathing then we know there's a lot of crappy sleep....with or without flagging of any events.

The bottom link....iffy as to it being asleep related...can't always tell easily. There's a big gulp of air (we don't take big gulps of air when we are asleep) and then something that looks sort of like asleep breathing that is very brief. Is it maybe a sleep onset central???? Maybe. Then after it the flag the flow rate isn't easy to evaluate. IMHO

The top link....I don't think sound asleep for the first 2 but maybe asleep for the last one. Again maybe sleep onset?
I don't know for sure. I have this sort of thing myself on occasion and I can't always tell for sure.

Again...I think you are likely having a mix of arousal related central apnea flagging and some asleep central flagging.

Since there's really not much we can do about centrals except maybe play with EPR (which only helps IF EPR is a factor in the centrals) I still say work on what we know we can fix with the machine tweaking and that's OAs and hyponeas.
And also give yourself some time to just get adjusted to and sleeping with the mask and machine....and THEN see what is left that might be a problem.
If an OA causes the arousal which then causes some arousal/awake central flagging or again some sleep onset centrals...then what we need to do is deal with OA/hyponea causing the arousal.

Thanks, I feel like I replied in the middle of the thread like on a different page, but assumed it would land at the end of the thread anyway so I don't know where it went. I am really terrible with a laptop and so much more used to phone. But for Oscar I will use the laptop :)
