Oscar Data Interpretion Alee

Post by alvinglee1 » Sun May 15, 2022 10:12 pm

60 year old male, 185 pounds. Fairly active with golf, gym, bike, travel.
Started using cpap 9 months ago. Resmed Airsense 11, airfit p10 nasal pillows.
Have been using the machine fine since the first day. Was breathing through my mouth to start but have solved that problem.
I have always been able to fall asleep in 5-10 minutes and cpap has not changed that.
My energy and weight levels and sleep duration are the same before and after starting cpap.
I do notice that I have less of a headache or brain fog when waking up now than before cpap.
I'm not real happy with my sleep Dr. - feels like a business or money grab rather than a great resource for information/health. I'll likely look for someone new.

Can someone please help me interpret my oscar data?
I believe that the events and AHI are good.
My concern is the leaks - are these ok or do I need to reduce them? I believe the air pillows are on all night and I don't hear them leak but the Myair app reading usually lists that as a bad score - (I know it's not too scientific).

Thanks for any and all comments.
Al

My guess is that you are still mouth breathing. The leaks are considerable, but they don't seem to affect your sleep or your AHI. Your machine can compensate for most of your leaks (up to 24L).

Either mouth breathing or mask movement leaking.
You could tape your mouth shut for one night and see what happens to help you figure out the cause.

The problems with large leaks is that if they are big enough it might be masking impaired therapy if the machine is missing events. In other words the nice low AHI might not be so nice and low.

In general the machine can compensate for leaks up to 24 L/min without impacting therapy itself but above that things start getting a bit iffy in terms of the machine responding appropriately and recording accurately. The further you go above 24 L/min the iffier it gets. So it something that is more gradual in terms of leaks being a problem or not.
It's been my experience that above 35 L/min things really get iffy with the machine's reporting.

In your situation (at least on this report) the large leak is maxed out at around 30 L/min and most likely the AHI is probably accurate. If you are sleeping soundly and the leaks aren't disturbing your sleep and you are feeling great during the day then I don't know that the leak is all that much of a problem but it is big enough and prolonged enough that I would at least try to figure out what is causing it and try to reduce the amount of large leak time if at all possible.

ResMed won't show Mr Frowny face for mask leak management until you spend over 30% of the night in large leak.
You barely make it over that line in the sand with 31% of the time over 24 L/min.

So is this leak bad enough to worry about???? Probably not in terms of affecting the machine's ability to do its job as long as it stays below 35 L/min and it's not causing sleep disruptions and you are feeling good during the day.

Yes, it is going to affect the MyAir sleep score but scoring 100 doesn't guarantee that you feel those good numbers either so I don't know that I would worry too much about the sleep score as long as I was sleeping good and feeling good. After all the goal is to prevent apneas from forming and sleep good and feel good the next day.

Personally....my own line in the sand is the 10% mark for time over red line. I would try to reduce 30% down to 10% if at all possible. Below 10% of the time I just shrug my shoulders and move on.
You get to choose your own personal line in the sand based on how you sleep and how you feel.

I will work on getting the leaks lower. I had bought a chin strap that I used for a few nights but I'll try it again and see how the graphs change - or tape.
Thanks for your quick and detailed responses, I appreciate it.
