Mild apnea/UARS/ASV (CPAP triggers centrals)

I'm new to the CPAP world -I was diagnosed with ADHD early last year, but after going to a psychiatrist for a bit they seem to think that either I just have sleep apnea or that apnea is exacerbating symptoms I have. I wake up absolutely exhausted every day and honestly the less sleep I get the better I feel. 6-7 hours feels tired but 8-11 makes me feel like death; stimulants keep me going but even right now I feel awful, definitely worse these past couple of weeks with it being cold and dry. I'll be honest in saying that with my wife and I being graduate students that we don't really have a lot of money to throw around right now, I've been trying to set up an in-lab sleep test (my MIL got a false negative for an at-home with a later lab test saying she has severe OSA) but this is now month four of trying to get into an appointment. My insurance is picky and my local and ONLY hospital is absolutely terrible with no other local places doing these tests.

That out of the way, I found a (seemingly) lightly used Airsense 10 Autoset that I picked up off craigslist for 40$ with 300 usage hours. The silicone smells slightly like smoke, but judging by who I picked it up from, general area I was in, getting a nebulizer in the bag with it and noticing that the air coming out of it doesn't really smell like anything...I get the vibe that this was from a recently passed away relative of the man and was only stored in a smoker's home afterwards and wasn't used by a smoker. I'm waiting on an order for a hose, bleep mask (no script) and filters...I guess I'm not sure what to do thereafter with settings and further cleaning -I still have every intention of getting a test done and I'm still on the 'list', but I'm tired of being tired all the time and really want to at least try the machine.

EDIT: I now see there's a new user section, I am not running on all cylinders today.

If it were me, I would set it on auto min pressure 7, max pressure 10. Check the Daily Details the next day in OSCAR. Post a screenshot and members will help you with interpretation and settings optimization. (Youtube videos explain how to get into the clinical menu to change settings and see reports on the machine display.)

By the time they get to you, you may not need them. Except, you might have to go through it to get them to write scripts for masks and other supplies to be covered by insurance.

Max 20, let the machine do what it can to alleviate apnea, absent a compelling reason to restrict it.

You got a great deal on a great machine! Starting with a minimum of 7 makes sense. I would add a recommendation that you set EPR at 3. EPR lowers the pressure when you exhale, which makes the experience more comfortable for many people. Start with a middle-of-the-road humidity setting (3?) and then move it up or down depending on your preferences.

As for the maximum, you can do as pr suggests and set it at 20 so you can see where the machine "wants" to go. You can also set it lower if you're not sure you want to experience higher pressures right away. If your pressure winds up plastered against the lower ceiling, you'll know you should raise it higher.

Please get an SD card of 32 GB or smaller to put in the slot in your machine. Makes sure you have a way for your laptop or computer to read it (SD slot or plug-in SD reader). Download the Oscar software (see link below) and use it. This is an invaluable tool for self-titrating your settings. You can take screenshots of your daily charts and post them here so we can offer our advice based on the data we see.

The bleep is a wonderful mask option, though it's a little fiddly. Be sure to wipe your skin with alcohol preps before you stick on the ports. (At my local megapharmacy, they're in the diabetes supply aisle.) Watch the video on the bleep web site -- it's at least as helpful as the written directions.

When you have the hose, filters, and mask, try using the machine someplace outside your bedroom during the day or evening while you do something mildly diverting, like reading or watching TV. This will help you speed up the process of getting used to the novel experience.

One last thing: if the bleep is too fiddly for you, you can buy mask PARTS without a prescription. If all the parts for a particular mask are available on line, you can assemble a full mask. I don't remember which masks you can do this with, but if you poke around on amazon or cpap.com, you'll be able to figure it out. You might start by googling "cpap mask parts."

Skin prep/cleaning is absolutely critical to success with the Bleep.
So follow the directions meticulously. I have been known to try to cheat and bypass the face washing part and just use alcohol wiping...and I end up with the adhesive coming loose in the middle of the night.

Thanks for the advice everyone! Trying it this evening for the first time. Will follow the Bleep application procedures closely and probably attempt a 7-20 pressure level. I thought I had a smaller SD card, but only have a 128gb so I'll have to wait on that front. Honestly there is a bit of a smoke smell, doesn't bother me much (now since it's not on my face) but will probably run it outside or get some ozone spray. I really wanna replace the silicone but can't find anything about that inner seal where the humidifier meets the machine...c'est la vie.

Got it! Got it! I've seen conflicting things about this and other forum posts that mention not using it for a good long while and to let it air out. Was looking at that ozium spray -though I've used a machine before when airing out a musty studio. I'll stick to running it outside for 24+ hours...Will the cold prevent the smell from dissipating? It's above freezing but still hovering mid 40-s the next few days.

In Kansas -only a little warmer than my family in Nebraska :wink:

From what I can gather, you'd spray an amount of it onto a cottonball and place it near the inlet filter. No direct liquid getting into the machine -only the vapors as it breaks down. Still potentially less than ideal but I'm still doing research on the whole process. Been mentioned a few places to good effect, but it's not a substance you'd want to directly breathe in regardless of being an 'air freshener'.

Having de-stinked a machine or two, I'll say that you might consider disassembling the thing, and boiling the silicone in some baking soda water (https://www.cpaptalk.com/viewtopic/t734 ... ml#p674729)

the plastics and the foam are easy to wash with some dish soap, and/or wipe down with alcohol... but smoke soaks into the silicone, and isn't easy to get out.
