TALLY LOG - Did Philips repair, or replace your machine?

It would be great to get a broad view of Philips 'response in the CPAP recall process.
So we can dig thru the data and make sense of it, please use a CSV format to log your data in 1 line, as follows:

QUESTIONS:
__which machine model did you had a claim on? __Model
__was your actual own machine returned repaired? (Yes/No)
__did they send a refurbished machine? ("Refurb"),__Model?
OR:
__did they send a new machine? ("New"), __Model?


SAMPLE ANSWER LOGS:
For receiving a refurbished machine=
SystemOne BiPap,No,Refurb,Dreamstation2 BiPap

For receiving a new machine=
SystemOneBipap ASV,No,New,Dreamstation2 ASV

For receiving your own machine back, repaired=
Dreamstation2 BiPap,Yes,,

(there are 4 answer items possible per log, so to fulfill the CSV usability, 3 commas are needed on each line to separate the answers, even for missing answers).

If you have more than 1 machine claimed, each needs a new log of its own, on a separate line'
Please do not add other comments in this thread, which will cause much work to sift out, when tallying the data; the answers we hope to get are 1-line log responses. Thank you very much for your contribution!
