It is important to note that not even Dr. K knows why off-label use of ASV (as in, for UARS or insomnia) seems to work better than other forms of PAP for some people. He has theories. Good ones. But only theories, really. The evidence is mostly anecdotal and collected in conjunction with a lot of direct counseling and attention that may account for much of the difference often cited between ASV vs. cheaper forms of PAP for treating things other than outright central sleep apnea.
"Treatment gains with ASV were 150% greater than with CPAP. . . . Research is warranted to determine whether the efficacy of ASV technology derives from facilitating increased PAP use, decreasing insomnia, enhancing objective sleep quality, or other beneficial effects that impact daytime impairment." --

How much credit should go to the nature of the machine vs. how much of the credit should go to the nonstandard protocols and subtle nuances of the added attention given to patients trying K's methods in clinical settings is a matter of debate among some in the industry, as I understand it.
Personally, I am a big fan of Doc K. I am not so much a fan of patients opting for ASV on their own for experimentation in dealing with what may or may not be UARS. Hey, just me. I'd rather experiment with an autobilevel first to see how much comfort I could get from that. Or high-pressure single-pressure straight CPAP. If no success, then, and only then, ASV for UARS or insomnia issues. And not for reasons of cost--for reasons of logic. The changes in pressure that are a comfort and benefit to many OSA patients can be problematic for some UARS phenotypes, I believe.

There you go, making sense again... There've been a few folks here that 'went the way of Dr K', to the tune of *thousands* (close to 10, if I remember right) out of pocket, and left disappointed.

That's very good insight, thank you for sharing all that info! I've certainly learned the internet can be your friend or enemy when it comes to our own research lol.

I know exhaustion is playing a part. It's just hard to get on top of my mind. I'm sorry to sound so weak. I'm a grown man at the end of my rope crying like a baby
Hi Mark, you are not weak, just exhausted and frazzled and desperate for sleep. I've felt like that too sometimes and it brought back memories for me reading your post.
Thank you for you're kind words!

If looking at S9 machines, the ASV to get is the VPAP Adapt, but only the REF 36037 model, not the older, less featured 36007 model. (REF number on label).

Do you think a chin strap or mouth taping is still needed with Bipap? Or is it pretty much the same.

Please don't use the term 'bipap', that's a trademark of Respironics.
Mouth taping or other anti-mouth leaking techniques are the same with bilevel and regular cpap.

Minepap 10, autoasv mode, defaults for everything else.
