I've just changed the nose piece on my Dreamwear. The other old one didn't appear to be worn out after 6 months but just recently my leak rate has increased and so have my large leak rates which were at zero. I could immediately feel the difference in the way the new nasal piece covered my nose and had no large leaks at all last night.
I find that the backstrap is only a minor player in keeping the nose piece in place. I just make sure it's not too tight. Adjusting the position of the silicone headstrap seems to determine what pressure is place on my nose. Too much pressure equals some pain and also, I believe, a less effective mask. I move mine slightly forward on my scalp.
