Bipap on doc's settings -- (IMAX 18, EMIN 8, PS 4) and (IMAX 14, EMIN 4, PS 4) -- gave me a ton of centrals. See thumbnail below.

I'm just trying to use the machine in CPAP mode so I can meet compliance goals until I get the bipap settings figured out.

When I use my auto cpap machine I'm typically at 4-7 AHI (pressure 9.5-13). I never see centrals like this. I haven't yet tried to use the bipap machine to simulate auto cpap, hence this thread. The clinical manual for the dreamstation shows an auto cpap mode, but it's not a selectable option in the auto bipap. That's what I'm interested in sorting out here.

I have a rare disease and use meds that cause breathing drive problems when I use them (not used in above night). My doctors anticipate this will increase with time so we're wanting to stay ahead of it by getting me used to bipap (or perhaps ASV) now. We didn't expect titration problems with the bipap.

I'd lower the ps settings.

First, I can see why you're interested in getting out of Auto BiPAP with all those centrals. One thing that I'd do is let the sleep doc who prescribed the BiPAP with these setting know about the sudden appearance of the centrals.

My understanding of many CPAP/BiPAP clinical manuals is that the company often writes one for the entire line of machines and somewhere in the manual there's a list of what settings are available for each model in the line.

I don't have a dreamstation. On the PR System One Auto BiPAPs, however, there was no "Auto CPAP" mode available. So maybe the DreamStation Auto BiPAP's also don't have an Auto CPAP mode.

So it is expected that in the future you may need to be able to set a different IPAP and EPAP pressure (basic BiPAP, but also a fundamental part of the ASV settings). But the 4cm PS difference in IPAP and EPAP seems to be triggering a lot of centrals.

Given that you have a rare disease and use meds that can cause breathing drive problems, I think that you really need to show this data to your doctors. And get some feedback from them about what is triggering all the centrals now that you are on BiPAP.
