Still extremely fatigued on ASV, help?

After two sleep studies showing AHI of 34 CA and 1 OA, I was prescribed the Aircurve 10 ASV, EPAP: 4, Min PS: 6, Max PS: 12.
After 8 months of consistent ASV usage, 8+ hours per night sleep with machine, with consistent AHI 0.0 - 0.4. I am still extremely fatigued, even more so than before treatment. Leak levels are very low. Flow limitation spikes occur throughout the night, but Sleep doc says they are meaningless, sleep doc is out of ideas and has no further help to offer. Consistently waking up exhausted, in a fog, with red glassy eyes, worsening memory problems, headaches. Adding Nuvigil makes slogging through the day bearable, but this is taking a toll. Any help would be greatly appreciated.

Vennechris, obviously there is a disconnect between AHI results and how you feel. see sI ome noticeable flow limitation, and might suggest an increase in EPAP min. Outside of the apnea treatment, have you had workups for testosterone and other possible sources? Are there any other health complications that might contribute to how you feel?

No other health issues outside of the centrals. Have had nearly a full blood workup, (no vitamin tests), thyroid check, heart echo, etc. Going to see neurologist next week to exclude that. Running out of options, was hoping you folks might see something I don't. Will try raising EPAP to knock out flow limitations.

I second the notion of raising the epap a bit, if it were me, I'd raise the maxps some, since you appear to be maxing out on that at times.

crazy flow limitations traces seem to be a feature of the resmed ASV machines, I haven't ever seen an explanation for it.

I have to admit, I'm a bit flummoxed. Even your tidal volume looks great. Hope the increased EPAP helps a bit.

Do any ASV users out there feel that the pressure changes prevent them from reaching a deeper level of sleep?
