IMO, the reason a wedge can help someone borderline OSA is that elevating the head can lessen the fluid in the tissues of the upper airway just enough to make the airway slightly less likely to close.
But the thing is, borderline-OSA people should get the chance to try CPAP instead of being convinced to try something that at best will barely move them out of the diagnosable category. The purpose of the studies is to allow them to try PAP, not to prevent them from trying PAP.
A borderline-OSA person with a cold or who sleeps too soon after a large meal or has some wine near bedtime or takes a Benadryl to fall asleep can have some rough nights and be sleepy enough from that to crash a car, same as a moderate-to-severe person.
