TO Pugsy.
The mask shoves the nasal pillows up my nose. I loosened them last night, but no matter what, it shoves them up my nose. Its the design I think. I am using small nasal pillows too. If I pull mask down a little the pain is relieved and there are no leaks, but there is no way to keep it down.

You might benefit from switching to larger pillows (med, etc). I found it easier to get a seal with a looser mask when I switched from medium to large pillows.
Nuance is a good nasal pillow mask if you're having nostril pain problems. The pillows have a gel ring that seals on the outside of the nostrils, and there's only a tiny bit of the pillows that actually enter the nostrils.
I just switched to the DreamWear (same as pwgphoto posted earlier), and It's even better, in my opinion--all the pluses of a nasal pillow with nothing entering the nostrils.

Nasal pillows that are trying to go up inside the nostril are either too small or the straps are too tight or both.
Nasal pillows should rest gently against the nostrils and not try to crawl up inside or push the end of the nose out of place....if they do then either wrong size or too tight or simply a poor fitting mask.
No matter if it is a nasal pillow mask like I use or the hybrid mask types that use nasal pillows.
Significant pain with any mask means either wrong size, incorrect fit or simply the wrong mask for the person.
