Any chance you can get an appointment with a real ENT specialist and bypass the Urgent Care docs?
Not that there is anything wrong with Urgent Care doctors but sometimes when things aren't improving with standard treatment then it is time to move along to the specialist.

Yep, you got it. Poor design alright. It does the exact same thing to me - the
straps folds my earlobes up and it is very painful for me. I push the thing down, b
but when I do, the nose part does not fit right - and I need a petite size for my small
nose. I'm going to make a video on this and sent it to PR.
Have you heard of these?

Is it the outer ear that's hurting or deep down inside? Does it feel like pressure in your ear like if you hold your nose closed and blow?
The CPAP pressure can go up your Eustachian tube and make your ears pop, but that normally shouldn't hurt that much.
If it's pressure in your ears, an Oracle Oral mask will keep the CPAP from blowing air into your ears as long as you use it without the nose plugs. Check the links in my signature line for more info. I found the mask uncomfortable in other ways.
If it's the straps going over your ears, you need a different mask or at least different headgear. There are several nasal masks that don't hit the ears.
