failed first night of VAuto BiPAP - OSCAR data for review

my sleep test said pRDI=8.9 with lots of heart rate spikes, especially during REM

Picked up an Aircurve 11 VAuto and ResMed AirFit P30i nose pillow and used it for a couple hours during the day yesterday. Comfort level was OK but I was unable to fall asleep with it, after about three hours I gave up.

I had my mouth taped closed, but when my muscles relaxed right as I fell asleep, the air pressure would force past the tape and inflate my cheeks, waking me up.

My settings were:

Mode: VAuto

IPAP: 15

EPAP: 5

PS: 4

Trigger: medium

Cycle: medium

Ti min/max: 0.3-2.0 seconds

No ramp-up

What settings could try tonight?

Welcome! Keep using the mask and machine during the day or evening to help you get used to it. You don't have to try sleeping with it right away.

Did your doctor prescribe the VAuto machine? And did he prescribe the settings? If yes to either, do you know why? I think we could make some suggestions about revising your settings, but it'd be good to get answers to those questions first.

my sleep test report made a very generic prescription of 4-20 cm CPAP so I'm trying to optimize based on more detailed data available

I made some adjustments last night and was able to tolerate the machine until 4 AM when my nose got sore and I removed it

My wife reported that she heard more leakage/whooshing sounds, but I'm not seeing any leakage data over 24 L/min?

I've also attached my Wellue O2 Ring data. Can't figure out why my pulse rate repeatedly spikes like that; do you reckon it's motion-driven?

Also, could you post a link to your SleepHQ charts? That way we can poke around in them ourselves to see what's what.

Leaks under 24 could easily be audible to your wife. And for a sore nose, you might try a little Lansinoh. It's made to make nursing more comfortable, so you might find it in the baby aisle at your drugstore. At night, you can use a tiny dab of Lansinoh for comfort, and it may provide a little bit of adhesion for your pillow mask.

I bought the machine out of pocket based on my sleep study results and I'm making adjustments myself - there is no doctor involved

Many thanks for the information and the link. I think your settings are fine, but you're having a lot of arousals. (You can identify arousal breathing because it is deeper and messier-looking than regular asleep breathing.) With just a little bit of luck, you'll have fewer arousals as you get more used to the machine. For many of us, it can make for some crummy sleep at first.

Are you taking any drugs, either prescribed or recreational? Do you have any health issues that might be affecting your sleep? Are you under special stress these days?

yes, agreed I'm experiencing lots of motion and heart rate arousals, though they don't trigger hypoxia. Hoping to get those tamped down

No drugs, no significant stress, no health issues. Just trying to get my sleep optimized - my bloodwork indicated low testosterone and prediabetes despite healthy BMI and exercise routine. I'm think they are caused by the disturbed sleep.

I will try to get more consistent with the CPAP machine; the mask is irritating my nose so I took a break for a couple nights.

Low testosterone and lousy sleep can exacerbate one another. See this:

For you,, here's the take-away: "Testosterone deficiency may have a deleterious effect on sleep quality that may be improved with testosterone replacement."

I'm betting that you'll have fewer arousals as time goes by, but you might also want to talk with your doctor about supplemental T.
