What is going on here?

Post by tiredNorlando » Wed May 17, 2023 4:58 pm

I posted earlier in the month about being tired again, feel like I sleep through the night, but wake up exhausted.. I am attaching two screen shots , one from before my suggested pressure increase and one before..

I am at my wits end, tried raising min pressure, max pressure.. was also told to try fixed pressure, but not sure what pressure to even try!

Can anyone see something in my charts that looks weird? any suggestions would be GREATLY appreciated!

kind regards

You left your minimum pressure at 11 cm and increased the max from 13.5 to 14.5...is that all that you changed?

What is your primary complaint? Fatigue? Excessive sleepiness? What????

Sleeping position?? On your back mostly or on your side mostly or a mixture?

What have you done in terms of changing the minimum pressure? Anything?

Haven’t changed minimum in a while

I would suggest increasing the minimum to 12 and see what happens.

Might want to be thinking about getting with your doctor about the fatigue. Wouldn't be impossible for something else to be going on.

The report looks a little "busy" to me but not horribly so. Some clustering of various events which is probably related to your going from your side to your back where you need a little more pressure....that is of course assuming you were asleep when those events were flagged.

Is it enough to blame your fatigue on???? Dunno, like I said it's not a horrible report but it isn't an entirely clean report either. It could stand for a little clean up or reduction in some of those flagged events.

If you increase the minimum pressure a little and things don't change in terms of the data reported as well as how you feel then I would certainly suggest getting with your doctor and having a work up for the fatigue. We tend to want to blame all our unwanted problems on sleep apnea stuff and think these reports are the gospel and can show us everything but in reality they don't and can't. There are a lot more causes for fatigue than just sleep apnea stuff.

Take any medications? Any change in dosage of any old meds...or added any new meds?
Medication side effects could be a factor as well.

No meds.. I will increase the min to 12 tonight and see how that goes.. what are your thoughts on fixed pressure? I saw a video of lankylefty showing fixed can be better than auto.

Thanks pugsy

I am not as big of a fan of fixed pressures for everyone like Jason (lankylefty) is for number of reasons.
Yes...some people do seem to do better with fixed pressures but it doesn't always work out so great if a person seems to need different pressures during the night for various reasons.
Sleeping supine vs side sleeping is one of those reasons...it's common to need more pressure when on our backs as compared to when we are on our side. Supine sleeping can worsen OSA and cause a need for more pressure. Same thing with REM stage sleep. I don't see a really big difference between side vs supine sleeping but I sure need a lot more pressure during REM than non REM. REM makes up about 20% of our sleep....I often will see pressures in the mid teens during probable REM stage sleep and the other parts of the night 9 cm might be all I need.
If I was going to do a fixed pressure I would have to use about 16 cm ALL NIGHT LONG instead of just 20% of the night.
It could be done I suppose but why in hell would I want to do that? That's a lot of pressure and it would greatly increase my chances to have aerophagia issues...not fun.

So Jason and I disagree on that one...we also disagree on EPR usages as well. He thinks no one should use it because it can possibly cause central apneas in a small number of people. I think that people should use EPR if they like it and want it and don't have centrals get triggered by EPR. Jason is less flexible...but he is entitled to his opinion and I am entitled to mine and there's no law that says we have to agree. Jason doesn't walk on water any better than I walk on water.

You are welcome to give it a try but you will likely need to use around 14 or 15 cm fixed pressure to best prevent airway collapses.

here is last nights results with min at 12... ahi was lower, mask leaks higher. felt worse today than yesterday, but I am going to keep the settings again tonight and see if I acclimate to them..

any ideas for a ffm that manages leaks better than an F30? I like the F30,and have had the best results with it, but leaks are leaks.... and I have tried many others....

thanks
