Help! my first follow up appt is Friday.

Post by Trainer4346 » Tue May 16, 2023 6:45 am

I have my first appt since starting CPAP coming up this Friday. I'm trying to get prepared with questions. I had posted this link but maybe it got buried in the other thread so I thought I'd try again.

Are there areas of my graph that look interesting/suspicious? ...that point to therapy adjustments?

Https://imgur.com/vPty8FV So here is a link to Oscar data. I think I have it all showing in the screenshot. The data look fine (to me) but all I know is I wake up all the time and MyAir says I don't, so I don't trust it. That and I just don't feel rested. I picked a fairly typical day. I can post other days if needed. I'm typically getting 6 ahi which is down from 28 in lab. That seems impressive but I feel worse on CPAP than without.

You need to understand the limitations of the machine itself and how or what it reports either to MyAir or the OSCAR software..
1....the machine can't really tell sleep status so it doesn't have any way to report wake ups.
2....the machine only measures air flow.
3....the machine may make use of the auto ramp function (which is what you are using) and increase the pressure when it thinks you are asleep but it is basing this "my human is asleep so lets increase the pressure" reaction to an educated guess based on flow rate that the human is asleep. It doesn't/can't offer sleep status at any other time other than the auto ramp function and it can't report to MyAir or OSCAR what it doesn't measure.

If you know you wake up a lot during the night then it doesn't surprise me that you aren't feeling well rested because just the fragmented sleep itself will mean you end up feeling crappy during the day because you didn't sleep well.
The machine doesn't know you woke up a lot though...simply can't measure sleep status. You are wanting information from the machine that it simply can't give you because it can't measure that data point.

Now figuring out what is causing the multiple wake ups....million dollar question and not an easy question to answer, much less fix, whatever the cause might be.

Do you have any idea what might be causing you to wake often? Mask issues? Leak issues? Comfort issues? You can't fix a problem until you identify the problem.

Oh...next time you share your detailed report image....remove mask pressure graph and include the regular pressure graph instead. Mask pressure graph isn't all that helpful unless we zoom in in some specific circumstances.

Thanks so much for responding. And clarifying the machines ability (not) to track sleep/awake . So...you ask if I have any idea what's causing awakening. That's my struggle. No idea really. I had awakenings before CPAP so I had high hopes they would stop once I started treatment. It's not leaking and I don't think it is mask issues. It's not as comfortable as sleeping without it but it's generally fine. So that leaves apneas or something else. I had hoped the graph would shed some light on this very question.

Do you take any medications of any kind, even OTC?
Any other physical or mental health issues going on that can mess with sleep??? Pain, stress, anxiety, etc???

In general 2 main kinds of insomnia.
Sleep onset insomnia which is where you have trouble falling asleep.
And Sleep maintenance insomnia which is where you have trouble staying asleep....and sleep maintenance insomnia can be compounded with sleep onset insomnia if a person has a lot of trouble going back to sleep when they wake in the middle of the night.

The list for possible causes of either kind of insomnia is quite lengthy and sleep apnea is but one of the items on that very long list but cpap therapy only fixes insomnia caused by airway related issues. It can't fix insomnia issues caused by other stuff.
