Machine arrived. Now what?

Post by onward60 » Fri Aug 05, 2022 8:00 am

My sister ordered me an Autoset 11 because she has more money than I do and was sick of watching me deteriorate. (Long distance watching. She lives on the coast. I'm in the Midwest.) As it turns out, my brother and both of my sisters have sleep apnea. It's a genetic thing, something to do with the shape of our chins and throats, she was told. But they have better health insurance and/or more money, so have been treated. It took me years to even get a sleep study. And then I failed my first trial because I had such a hard time getting a proper mask during the pandemic. Now that things are more opened up, I went back and got a new script only to find out there is a waiting list for the machine I had before that my insurance covers. With the previous machine, it was set up at the DME. This one came in a box and I am supposed to know what to do with it. I am so exhausted, I can't even face opening the box. Is there an unboxing/setting up video for this sort of thing? Or does anyone have any advice? Be nice. I am very anxious about this and utterly exhausted, practically brain dead from 12 years of untreated sleep apnea.

BTW, I tried to update my signature and apparently failed. Will try again.

Go here and download and save and read the clinical/provider manual
https://media.resmed.com/sites/3/202106 ... ER-ENG.pdf

For additional instructions and video access to instructions.
https://www.apneaboard.com/resmed-airse ... setup-info

We have no idea what pressures to start with so try these
Mode....AutoSet mode...auto adjusting
Minimum pressure....7
Maximum pressure ....20
EPR...turn it on and set it to full time
EPR setting itself....set it to 3

AutoOn....set it to On
AutoOff...set it to On

I can't remember the others but these are the main ones for now.

Get OSCAR....you will need it.
If you don't have a SD card in the machine go to WalMart and get the cheapest generic SD card you can ger.
There MUST be a SD card in the slot for you to get the detailed data that OSCAR can give you.

Thanks, Pugsy. Does the DME help with that?

Edit: The prescription says: Use 5 of 16 cmH20 for all sleep. Is that the pressures?

Definitely not - tho' plenty think it's them! Your MD prescribes, but again, while they 'should', they so often don't know much and that's why people end up here needing tweaks, etc. after a few nights of using Oscar software getting posted here (using default settings of 4 and 20) and realizing things aren't working well for them. Once experts here see Oscar they can then recommend changing pressures, or adding other features, even possibly suggesting machine changes (there are diff. types depending on your results).

Also, others can prescribe - as in sign scripts already titrated for you - such as dentists, nurse practitioners, etc. etc.

Thanks!

I just edited while you were also writing. The prescription says 5 of 16 cmH20.

I have no idea how all this software stuff works. My daughter will help me. I am practically brain dead from lack of sleep.

"Attempt two. Haven't decided on a mask yet."

For your signature look in the "A's" for Airsense to find the ResMed Airsense 11 if that's what you have. And are you saying you don't have a mask yet? You'll need a mask to use with your machine.

Are you sure it said five OF sixteen, or possibly 5 and 16... which would make more sense if talking of min and max pressure settings.

Yeah, it says "of." I wonder if it means "to."

Download the MyAir app on your phone and it will step you through setting up your machine. Edit-I see Pugsy gave you settings suggestions already.

My daughter claims she set it up. I haven't tried it yet.

My daughter says I don't really need Oscar and a SIM card to start practicing with it. Is she right? The last time I used it during the day first to get used to it. I am probably a long way from being able to wear it at night.

For practicing while awake you probably don't need the data but if/when you try to sleep with it then you may need it because in that data might be a clue if you are having some sort of problem.
You do have to be asleep for the data to mean anything.
Awake data doesn't mean much.

@ Onward60 -

Pugsy is right as always. While it is true that wearing your gear while awake may help you become accustomed to it and train your brain to view it as a friend not as a threat, the awake data is pretty much meaningless. Your daughter is right in that limited sense. When you start to use it at night however, the SD card will be essential in capturing your data and OSCAR will be essential in reading it. Data will help the experts recommend pressure settings for you. 5 to 16 may happen to be right. Likely not. 5 is too low for most adults. So get an SD card and OSCAR, down your data after a couple nights, and post it here so the experts can evaluate it and advise you accordingly.

Well, I am a LONG way away from using it at night. My machine is still off-gassing a plastic smell. I am running it 8 hours/day with no mask on it to try to get rid of the odor. It's not the hose. The hose doesn't smell. The smell is coming out of the machine itself.
