Let's get clear about what we are calling things to avoid confusion.

BiPap....that's a Philips Respironics model name for a bilevel machine...bilevel just means 2 distinct pressures.

There are several different model bilevel machines...and each model might address a different problem or do something different.

Pressure support/PS is the difference between inhale and exhale.

ASV is a type of bilevel machine that can auto adjust as need for both obstructive apnea and central apnea.
It has a back up rate available so that if you don't breath enough then the back up rate will kick in and force you to breathe.
The PS will auto adjust as needed as well. This adjusting PS is what is needed to treat a central apnea and force you to breathe.

Regular bilevel machines...like the one I use (see link in my profile) don't have a back up rate and the PS is fixed and can't auto adjust. So a regular bilevel machine can't treat central apneas.

The bilevel S/T models do have a back up rate but they are totally fixed...no auto adjusting of anything.
They work more like a ventilator. Usually people have a really high PS because they have nothing but a lot of centrals to deal with. So you get a really high PS with every breath instead of only when you need it. Not much fun.
Pretty sure you won't like it.

Thank you, that clarifies things quite a bit. The practice I was going to offered cpap/bipap/st titration only -which I still can technically get ordered, but I guess I didn’t realize just how similar they all are. Thought ASV was a whole different type of machine, but it sounds like it’s just a less lazy implementation of an S/T?

ST....fixed pressures only. Can't auto adjust anything. Forces big PS with every single breath and people find it a nightmare to get used to. Not particularly good for obstructive apneas. Everyone who comes here with one just hates it and usually ends up on ASV.

ASV...auto adjusting EPAP and IPAP for obstructive apneas and auto adjusting PS for central apneas and you only get the big PS for centrals if/when you need them and you are asleep and its usually for only one breath. Much, much easier to get used to. It works like an auto adjusting mode machine for the obstructive stuff and then if/when centrals happen the at that time it will increase the PS to force you to breath. I like to think of it as more of a plug & play machine. So much easier and better IMHO. It only increases pressures when you need the increases. If nothing is happening then it doesn't do anything except blow gentle air at the minimum settings.

Managed to snag a fairly low (3000) hour non-smoking DS960P ASV. Gonna attempt to titrate it myself -a new job means I don't have any insurance at the moment so I'm going into this blind(ish).

Changed my thread title as I'm now on an ASV, having some issues dialing it in -it's being super aggressive with timed breaths. It's either that or my centrals are going crazy and it's doing its best.
