Exhale is dropping off a cliff

Post by ShallowBreather » Tue Apr 19, 2022 7:16 am

Hello

I was recently diagnosed with centrals and given an AirCurve 10 ST. The setting were initially 16/6 with PS 10 and backup rate of 11. When I was unable to get to sleep because of the "kick in the face" that I was feeling from the sudden burst of air, they made some changes to the settings. Currently, I have:
IPAP 15
EPAP 9
Resp Rate 8
TiMax 2.0s
TiMin 0.3s
Rise Time .400 ms (This was one of the changes they made)
Trigger Med
Cycle Med
No Ramp

When they increased the Rise Time, it really helped to remove the kick in the face feeling.

But, now I'm noticing the opposite. When I move from inhale to exhale, the bottom just drops out from under me. It's like falling off a cliff. Sudden and drastic. Is there a setting that will help with that?

I guess I should add that I was able to keep the mask on for about three hours last night, and I only had an AHI of 3.18, all hypopneas. No centrals.

It would help if you'd keep all your posts in a single thread.

And no, there's no inverse rise time setting.

Just a suggestion based on a guess of what may be bothering you in this case: try increasing TiMin, it will hold your IPAP longer, so even though it won't decrease the pressure setting time, it may make the drop more tolerable. It worked for me; my TiMin is at 2.1 sec (the maximum), but I should also mention that my TIMax is at 4.0s (also the maximum) as my vital volume is rather large, and TiMin <= TiMax.

Interestingly enough, moving from the Simplus full face mask to Swift FX removed the “falling off the cliff” feeling. It now breathes fairly naturally.

Could it be the one-way valve in the mask causing that? I think maybe.
