Quick Senior Design Survey for UNI

Good Evening,
My name is Iyanuoluwa Ojediran I am a student at The University of Texas at San Antonio and my team and I are working on improving sleep quality with the usages of a CPAP’s and it’s mask. For our Senior Design project we have created a survey to gather and accrue data to better understand CPAP Machine issues. It would mean a lot if you can complete the survey below! It does not ask for your name, personal information, or emails; simply just a few yes or no and scale questions about the CPAP machines and sleep apnea as a whole.

no good ever comes of giving your e-mail address to a stranger on things like google forms.

No personal identifying information is required to participate in the survey.
Age is as personal as it gets. That's it.
No where does it ask for e mail address.

I would NOT have approved the above new account post if it had asked for any personal identifying information.
Just a few days ago we had someone else you did require email address and I didn't approve that new account post.

the very first thing it wants is your google account which is an e-mail. maybe the person can see it, maybe not. google can surely add it all to their profile of you for targeted ads. i guess it depends on their definition of not shared. just my opinion, everyone has one.

I ignored the the google required address. Took survey. Submitted. No problem.

I think the google account requirement is if you want to (for some reason) take part of the survey and then want to finish at some later time. It saves your work so you can return.

It is not required just to take it and go on about your business.

No...No....No.

You do NOT have to sign in to your google account at all to participate.
Sign in to Google to save your progress. Learn more
Signing in would enable someone to SAVE THEIR PROGRESS if for some reason they wanted to do that.

I don't even have a google acct....I took the survey...hit submit and got thanked...and no google acct was involved at all.
A non issue.
Now I didn't save my progress or the fact that I took the survey...but who cares.

I do admit that the Red "required" thing right below the Google sign in thing is a bit misleading....but no where is there anything required in red other than in that unfortunate placement of something that might be required.
It does sort of hint that using google acct is required...but it isn't.

No where did I ever have to submit my personal email address to take that survey.

I am curious about the reasoning behind the question, "When did you receive your current CPAP prescription?"

My first prescription was nearly 15 years ago. For requirements from my DME, I get my doctor to give me a new prescription each year. The prescriptions always say the same thing.

By asking this question, were you trying to find out how recently the prescribed pressures or prescribed machine changed?

For years, many cpappers have been using mouse pads under
the cpap machine to prevent noise transmission to a nightstand.
Mouse pads were once easy to find--not so much now.
Perhaps an engineer could develop an alternative, insulating pad that
is easy to clean, produces no lint, etc. A sellable gadget--no RX required.

Alternatively, an online, brand neutral sizing program based on 3d imaging
would spare many from getting stuck with expensive masks that DO NOT FIT.
This would make public service points galore for you and your school.

Oops. I just noticed you are in Texas.
Scratch the public service idea, if it gets you in trouble.
