Dreamstation to Aircurve 10 VAUTO Settings - PS help needed
Hello,
After some help from this wonderful forum the screenshot below is a bad day. As the title implies I have a Dreamstation but need to switch to the Aircurve 10 VAUTO because of the recall. My sleep doc is useless and wants another sleep study before writing for a new machine, even with it being 5 years old. Most providers seem to be dropping the ball with this recall, side issue.
I went ahead and bought an Aircurve 10 VAUTO and now I need some help with the settings. To me the biggest change is that the PS does not adjust like the Dreamstation. I am looking for a good PS to start with. It seems my average is around 2 so I was going to start with 1.6.
Any recommendations you have would be great as tonight I plan on using the new Aircurve for the first time.
I will be monitoring this today so I can provide updated info as needed. Thank you in advance for any help.

The 95% numbers just mean you were at OR BELOW that number for 95% of the night.
It does NOT mean that is where you were at for 95% of the night and it is NOT an overall average.
Try PS of 2 and PS of 3 while awake and choose the one that is simply more comfortable to you.
Given the looks of your detailed report I think a little more IPAP is in order and the easiest way to get that is a little more PS.
Try it for a night and see what the report shows tomorrow as well as how well you slept.
It does NOT mean that is where you were at for 95% of the night and it is NOT an overall average.
Try PS of 2 and PS of 3 while awake and choose the one that is simply more comfortable to you.

With a PS that low, you don't even *need* a bilevel machine, but, you've got one, so set the PS to 1, or 1.6, that's an insubstantial difference. feel free to play with it as you like, I wouldn't go past 5 though.timeforagorism wrote: ↑Mon Sep 06, 2021 7:27 amHello,

I went with the PS of 2. Breathing does seem much smoother with the Aircurve as compared to the Dreamstation. Usually that pressure spike on the Dreamstation would wake me up.
Are there ever any concerns about too much pressure?

Supine sleeping and REM stage sleep are both well known to worsen OSA and cause a need for more pressure.
Given the pattern to when your increases seem to happen my first thought is REM stage sleep.
Google sleep stages and take a look at the normal hypnograms you can see when REM happens and about how long we are in REM.
My own OSA is documented 5 times worse in REM than in non REM and I often need 6 to 8 cm more pressure in REM. I have seen spikes similar to yours myself.
And there is nothing stopping you from having a double whammy with maybe you are on your back and in REM at the same time. It sure wouldn't be impossible.
Yes, sometimes people do limit the max IPAP but they only do that if those higher pressures are causing problems that are worse than a handful of OAs might cause.
Are those increases causing you to wake up or causing you to have air in the gut issues?

This does line up with when I usually hit REM sleep. That was typically the time I would have more issues. I seem to be tolerating the increased pressure, none of the issues you mention above. The pressure probes on the dreamstation are a 1.5cm pressure increase over the period of several minutes, they're gradual changes. Snores and flow limited breathing caused the pressure to bump up, opening your airway, which stopped the snoring and flow limited breathing.

Well this is a first. I feel like I slept pretty dang good last night. I had been having crazy nose itching with the Dreamstation, I almost think that was the particles. I have not had any of that nose itching after switching to this machine same mask and everything.
