The presence of the large number of centrals on your other sleep studies and the trial with the BiPap ST proves the need for the ASV machine.
Insurance companies will go more on "needs" than "does it work" thing. I doubt that they will put the sleep study under the microscope anyway. They normally don't ever see them. Everything is pretty much based on codes and what the doctor says is needed.

Oh. So if I understand correctly... even if this doesn't get me to a target Ahi of 5... as long as its less than the 50.3 Ahi I had with bipap st then that is what will be prescribed? If I understand correctly then that would indeed be good news.

I think that based on your tests and results, you will be prescribed an ASV machine and insurance will NOT bat an eye over it.

So, take a breath (no pun intended) - and wait to hear what will happen.

I think you will be on the ASV, at home, very soon.

I totally agree with Madalot.

Even if they didn't get AHI down to 5.. say they got it down to 10....that's still a whole lot better than 50.

Fair enough. That helps with my concerns. I know the study center / dme combo is usually a bad idea... but it seems like this one is doing right. Thanks again for all the support and insight.

That's why we're all here. When you hear, tell us what they tell you - and be as specific as you can. When they say you're getting an ASV, ask the DME exactly what machine you will be getting and try to find out all the settings.

Give us as much info as possible --

Even the 10 is not necessarily the best it can do; as, while doing nothing different,
my AHI went down substantially over the first 4 months.
I know myself that therapy can often improve with time, practice, and familiarity.

Exactly, CF. And, when you're dealing with the higher end equipment like ASV, it frequently takes a lot of time and tweaking to get the therapy optimized. One night in a sleep lab usually doesn't get you exactly where you need to be.

I was just using 10 as an example. I have no clue what numbers I got with the ASV as I have yet to get the results of that study back. They were kind enough to print out the bipap ST study for me, and I will post that shortly once I get it scanned and redact my info.

I don't have the test yet... but have confirmed with the scheduler that I don't need another test. Waiting on my primary care dr to review and write me a script. Pressures recommended weren't broken down over the phone... but its :

Confirmed that the Dme is in network. They have the resmed advanced av s9 or the respironics bipap av auto advance. Not sire which ill be getting.

Tell us wnich one you're getting, and how does it feel?
Personally I have the PR ASV, and I would really like to know more about the ST, can you describe how it felt?
Did it give you a lot of machine-triggered breaths? did it have a very regular rhythm and pressure, or did it vary a lot?

Yeah, I will definitely post more as it progresses. Nothing new yet. Still waiting on my script.

4better02. The ST didn't seem to vary. It had one epap and one ipap. It only changed if it noticed I quit breathing and then tried to pump enough pressure to "convince me" to take a breath. I closed my mouth while awake, trying to distribute moisture, and it seemed to kick on full force when I did this. The only changing it did was whatever the tech changed.
