Just curious if anyone here has been using an ASV for any length of time?

With my machine set at EPAP 8-12 and PS at 10, IPAP was going to 20.

With EPAP set at 8-12 and PS 17, it frequently goes to 25.

I am trying to figure out why it goes so high and if anyone else has one going to high pressures, and what you do to cut down on leaks.

are your ps min and max both set to 17 ?
then you will only get 25 for ipap

your settings shoudl be peep = epap min
ps max - limits the max ipap but yours is set to machine max so irrelevant
and ps min - this give ipap min = epap min + psmin
and then epap max - gives some EPR but limits it to 12 on yours even if IPap goes to machine max

with your epap 8 and ps 17 of course it goes to max 25
if you set your ps min to 4 it woudl run from 12 to 25 depending on your needs
lower your ps max to 12 and see if the leaks dont stop and your AHI is still about the same
taht would run ipap from 12 to 20 with epaps from 8 to 12

you are having hypops and it is trying to stop them

PERFECTION IS NOT POSSIBLE
you will have to compromise AHI sleep disturbance leaks CA etc to do the best overall

anyone who disagrees can tell my board cert sleep doctor and my DME who both say this is correct
PS max is 17, will have to check PS min - I think it's pretty low (4 maybe?)

I had PS max set to 10 and had a really bad night - more leaks and more hypopneas, so put it back to allow it to go to 25. I feel better with it allowed to go to 25 which is scary, that seems too high.

The leaks have not been waking me up (yet) but my mouth gets REALLY dry (nasal mask and mouth blowing open).

EPAP seems to be fine so far - I had higher EPAP with APAP set 12-18 with EPR 3.

Is my EPAP too low compared to my IPAP? Is the large pressure difference creating more leaks?

Actually with the S9 Adapt (which is what you are using) SleepyHead is pretty much spot on accurate with what it reports.
When in doubt use ResScan to verify.

Mr alphabet boy doesn't know what he is talking about in terms of SH and this ASV and the settings being reported.
2 S9 Adapts totaling probably 4 years worth of using SleepyHead and ResScan and I never found one time that SleepyHead didn't report what ResScan reported.

Besides...it's super easy to verify settings simply by going to the clinical setup menu area.

Apples and oranges.
You are using AirCurve 10 ASV
OP is using S9 Adapt (which is ASV) and any SleepyHead bugs were long worked out with the Adapt.
You can't compare SleepyHead Adapt setting results to AirCurve 10 ASV setting results.
I repeat....SleepyHead is Beta software and if you don't know what Beta software means go look it up. If you don't like dealing with Beta software bugs....simple fix...don't use it and quit your bitching.

Experiment as you are doing to find what works best for you. PS max of 10-12 is enough for me, and more than that just isn't necessary.

I would keep the max available to 25. It's there when you need it.
If you post a chart, you will get some opinions on it.

you are still an idiot, the machine will raise the pressure to what it needs to clear an event, a max available setting gives it room to move.
adjustment is normally with the min settings for UA, H and minute vent. It's the reason I suggested putting a chart up.

1) A doctor did not set or recommend any settings - this is a trial while waiting to see a doctor.

2) I was prescribed CPAP with a machine set to APAP. I had really crappy sleep and numbers so I kept raising the settings. Last settings were 12/18 and I was not seeing any improvement in my crappy sleep.

3) Since being on the ASV I am less tired (definitely not energetic) but am having a lot of issues with dry mouth and high leak rates (18% or so) from my mouth blowing open. If I could get the leaks under control I think I would be on the road to improvement.

Current settings are EPAP 8/12 and PS 4/17 and it goes to 25 multiple times per night. I have hypopneas lasting over a minute every night - probably because as the machine attempts to resolve it by increasing pressure, it triggers a large leak. Very aggravating...

I would raise the min epap till the H resolve. I would also use a full face mask while you are setting up the machine, it removes one variable. It would really help if you put a chart up.

I wan't going to bother putting up a chart until the leaks were resolved. With this many leaks the rest of the data is probably not accurate enough to give good answers.

Although if you want to see one I will put one up tonight. SH graphs work for you?
