Your doctor will send a prescript to the DME telling them at what pressure to set the machine. Oh, and one thing you'll find by digging around is that APAPs are billed THE EXACT same as CPAPs to the insurance. The only difference is the profit that the DME makes off the machine. What is an APAP? That, is an excellent question that any sleep doc should be able/willing to answer. So can/am I.
