Upgrading from CPAP to APAP, what configurations do I change

Post by john844 » Tue Jul 22, 2014 9:31 am

I currently have a 10+ year old resmed cpap, but am buying a resmed s9 autoset. Is there any specific configuration that I should to do before using the new machine for the first time or can I just use the default settings?

Thanks,
John

I advise setting it close to the settings you're used to... unless you're used to a cpap that starts at 4cm and goes up to whatever, then it'll likely be very jarring

you can set the autoset to cpap mode and the same pressure you're used to, or, if you want to try autoset mode, then set the lower pressure to a little less than what you're used to on your old machine (eg, if you're used to 12cm on the old one, set the min pressure on the autoset to 11.)

Do you know the pressure of your current cpap machine?

If you don't know just run it for one night open from 6 -20 and check the data.
If you know your pressure set the apap x-2 to x+2 where x is your usual pressure. This will let you see if x is adequate for your needs.

Great advice. It has been a long time since I changed the pressure. I think it was 8 or 9. I will check the old one and set the min and max.

I don't use the ramp feature on my current one. I like it to just come on, so I will replicate that setting also.

Thanks,
John

Thanks for the responses. Based on research and recommendations, this is what I am looking at so far.

Tubing Type
Mask Type
Ramp = Off
EPR = Full
EPR = 2
Mode = AutoSet
Max pressure = +2 from current cpap machine
Min Pressure = -2 from current cpap machine
Climate Ctrl = patient
Sleep Quality = On
Smart Start = On
Date
Time
Reminders --> Filter = 6 month recurring

Please feel free to add or suggest other settings.

I'd suggest -1 from current settings.

then look at your data with sleepyhead, and if you're spending all your time at min pressure, and you're happy with it, then either leave it alone, or drop it another cm.

if you're at 9, for instance, and you *need* nine, there's no reason to disturb your sleep by starting at 7 and having events till the machine ramps up to 9... being closer to where you're used to will result in less potential disruption.

Ok, that makes sense for the min pressure. x-1 it is. I didn't realize that the behaviour would drop to the min unless there were no episodes happening. The only time I could see needing less than my normal pressures is when I lay on my side, but I am used to them staying elevated today. I can deal with that going forward if it helps minimize episodes after changing sleep positions.

it starts at min and bumps up based ob flow limitations, snores, and repeated oa and hypop events

Do a few nights with the new APAP set to manual CPAP with the exact same settings as the old one, including EPR. See if it feels and works the same as the old one. Then when you start changing settings you have a good baseline to compare it with. Then start with small changes like 1 cmH2O pressure and watch the results.
