Simplus impressions and possible issues

Post by Juanefny » Tue Apr 22, 2014 8:00 am

Yesterday my new Simplus arrived in the mail from cpap.com. I slept all night with it and like the fit much better than the Quattro Air. I can move around a lot more and don't have to worry about the dying dolphin sounds from leaks. That said, last night I was very congested (have allergies at the moment) so I was mostly mouth breathing and I woke up twice and realized that the mask had apparently slid up and my bottom lip was actually below the bottom of the mask!
I had two sleep studies and used this mask and this size for both of them and never had this issue, so I'm wondering if I'm doing something wrong with one of the straps? Any help is greatly appreciated.Image

Additionally, I just got the sleepyhead data and my ahi was above 3.0. That's a first in the week I've been in therapy. While i got the leak graph lower than it had ever been, there were two very very large leaks, higher than anything I had on the Quattro Air. I'm guessing that may have been when I ended up with my bottom lip below the mask's edge.
Any impressions, ideas, tips are greatly appreciated
PS. I still slept great and I'd say better than on prior nights. I dreamed a tong last night!

You can turn off the AHI graph and the Mask pressure graph in SleepyHead. They aren't really needed. The AHI information on the left is more important and Mask pressure just mimics the regular pressure line.
To turn them off just go to Preferences/Graphs tab and remove check mark. This will allow leak graph to move up into view and you only have to do 1 screen shot.

Those 2 or 3 leak spikes....if they didn't wake you up I wouldn't get all worried about them. They didn't last long and only one was really massive and it was rather brief.
Since you already know what caused them once you get the fit perfected they should go away or at least not happen as often.
I can't help with fitting ideas..sorry..never used a full face mask of any sort.

Otherwise your leak line is to die for good...I rarely have a leak line as good as what you had last night.

Oh...I wouldn't worry about the change in AHI. It may or may not be related to the new mask. Might have happened anyway.
I see bigger changes than that on occasion and don't have any changes made to anything.
It's still a well within acceptable AHI number and the most important thing is you are feeling good.

The graphs I like to see are
Events graph
Flow rate graph
Pressure graph
Leak graph

I went to F&P SIMPLUS after four others in 6 months. The Tech set up the head strap much loser than my previous masks. Actually less than my typical baseball style cap tightness. Then the two side or cheek straps are also loser than my Quattro was. But the design of the mask keeps good seal without making my head feel like it is in a vice.

You may just move around a lot and dislocated the mask. I wouldn't worry at this point, especially if overall it looks like you are starting to get the benefits of the therapy. I think many mask users re-train their brains to reach up & readjust their masks without hardly knowing it. Then drop right back into sound restfull sleep.

Don't give up! Once you have the mask fit minimizing leaks, make little adjustments to the straps and give each adjustment a night or two for you to settle in. As I said, what I really like about the Simplus is how comfortable it is and no more raw nose bridge. Best wishes for you.

Don

Thanks Don! I also think the congestion may have been the reason I moved more today than usual and may have opend up my mouth more than usual too.. Over all I'm very happy with the SImplus and think it's the mask I'll primarily use going forward.

I have been using the Simplus since it came out a few months ago and love it........one thing that is different from other full face masks that I have used is the head gear has a strap that goes right over the top of your head...........if your mouth slipped out of the bottom you might look at that strap that goes over your head...........try looking in the mirror with the mask on and see if maybe you should loosen that top strap so that it is not "pulling" the mask up too high on your face............I loosened mine so that the strap coming from the forhead frame is going straight back and not being pulled upwards by that top strap over your head. Thats the only thing I can think of that might be pulling the mask upwards above your lower lip.

I'll be sure to try that tonight. Thanks!

Juanefny, being congested probably just caused you to open your mouth too wide and your lip slipped out. It has happened to me on a few occasions with different masks (including the Simplus). It probably won't continue, but, if it does, you can do a couple of things besides adjusting the top strap. You can wear a chin strap for while to train yourself to keep your mouth closed or you can rig a strap from the bottom straps of your mask under your chin. This strap does not have to be tight - just snug enough to remind your mouth not to open too far. There are a few examples of this type of strap on the forum. I think the last I saw was a made from a bicycle pants cuff retainer posted by Sir Noddinoff (I think).

Thanks everyone for all the great advise. I was not congested last night and never had the whole "mask in my mouth" issue, so it may have been that, but I also did refit my mask and ensured the top straps were not tight.
i don't have my computer with me, so I can't see what my leak line was, but my AHI was only 0.1 so I'm glad to see that go back to normal
