Getting my ASV soon What should I do

Post by sleepinow » Sun Apr 28, 2013 12:39 pm

Hey everyone I am finally getting my machine on Wednesday and was wondering if I should buy a different mask since this machine is completely different from my apap machine. I have the swift fx and found it comfortable to wear with the apap however should i change masks since ASV is a whole different machine?

And once I do get my machine what should I place the settings as? I cannot afford titration after this expensive set. Well I do not want something underpowered or anything overpowered. What would be a average setting that would be more easier to adjust then if I had a too weak setting or a too strong one. Like should I start off with a pressure of 6 rather then 3 or 10?

Asv is more complicated to set up. They have the min and max... and possibly pressure support. They are harder to get dialed in and lab titration is reccomended. Not saying it's impossible, there are more knowledgeable asv users on the board, and im sure some will chime in shortly.

If there are any cpap masks that will not work with asv, I am unaware of them.
The general consensus is that any mask should work with any machine.
If your current mask works with your face, keep it unless your pressure makes it unusable--only you will know for sure.

Hello Sleepinow,

By Wednesday you should have quite a few comments, expert advice and input to work with.

What has been posted so far about the masks not making a whole lot of difference between APAP and ASV, I have found to be correct also.

When it comes to initial settings, regardless of which machine you are getting, pressure support is generally added to your end exhale pressure or EPAP. If it were me and I wanted to have a base pressure of 6, I would set EPAP at 6 and leave the ps min and max at their default initially.

I recently had a patient in your exact circumstances and he did very well with his ASV.

Good luck. Please keep posting as to how you are doing.

That is good to hear

I am getting very paranoid that the ASV will not work for me. I do not know why but I think it is from the failure I had with my APAP which bothers me a whole lot. I could not even take a 20 minute nap because my centrals kept on kicking in. It was about 5 or 6 times that I noticed me not breathong in that amount of time

I would not worry about this. I had the same exact fear. Failing cpap, bipap and bipap st, I didn't think ASV would do it either. I bet you'll be fine

Just strap the mask on and try to seep with it. If you have trouble adjusting you could try using it while awake. This can help you so your more adjusted while sleeping. And will also add to your hours for compliance.

Have you had some kind of titration to determine your pressures...?

I did not have any titrations because I do not have the money for it. I am getting my machine on Wednesday so I was hopibg someone can help me. I did get my pressures set with my APAP however that did not do too much since I have more centrals than obstruction

What pressure were you seeing on the APAP and what was the AHI ...each event category?
It's impossible to pick out settings on the ASV machine because they are so many and so varied but maybe if we had an idea at what pressures you were using the APAP on...and how many centrals and at what pressures...maybe it would give someone a starting point.

You are going to need a pressure that will deal with the obstructive part of your sleep apnea to start with and there's a big range there.

It was a pressure of six I have never used it for far too long because it increased my centrals

Okay..you had centrals at pressure of 6...did you have any OAs or hyponeas at that pressure?
