New Machine - AHI off the rails

Hi All,

I'm new here as I came across this as I was searching the net to try to find some information. I recently relocated and my insurance changed so I had to go through a new equipment provider and last week I was setup with the Respironics Dreamstation after having used the Resmed Airsense 10 Autoset since March '19. Since I was started on the Resmed after a take-home test, my pressure was set between 6/12. My AHI for the test was 36. After I started using the Resmed, my app was saying my AHI was always <5, with maybe a night or two a month it would be between 10-20.

Since I've started with the Dreamstation last week with the same 6-12 pressure, same mask (P10 pillows) my AHI has been > 30 each night I've used it. I've attached the screenshots from OSCAR for last night.

I'm a software engineer, so my initial thought was that the algorithm between the 2 machines is different -- but should it be THAT different? My new family doctor here is who set me up with the new machine so I'm wondering if maybe I should see a sleep specialist here like I did previously? Could it be something wrong with the new machine? Any thoughts/suggestions would be greatly appreciated! Happy 2020.

Do you feel like you are sleeping better -- or worse -- than before? Were you awake much of the time?

Honestly, I feel like I am sleeping worse with this machine. I don't like the way it feels when I'm breathing compared to the Resmed.

It's quite different. Any chance you can get an Autoset instead?

Maybe turn off flex and see if that reduces your AHI. Have you changed altitude as part of your relocation

I would start by working back through your Doc and DME. It is clear that the current setup is not working well!! While folks here can (and will) happily work to improve your settings and outcomes, I would also work on seeing if a different machine is available. I also went from ResMed to Respironics (for a week) and then back to ResMed. I much prefer the "feel" of the ResMed. You may have the ability to swap brands for the first week or month -- now is the time to check !
