It's a hard topic to really understand, but 30 pounds in nine months, all other things being equal, would have me at my doctor's extremely quickly - as in, five months ago. I'm no more a doctor than most of us here, but that kind of rapid weight gain would scare me.
The question that comes to mind is the precision of "all things being equal." Thirty pounds in nine months would be a surprise and be hard on the body and all that, but in reality - it's only about 400 calories a day. That's about three slices of bread, a couple of beers, a couple servings of fruit juice. It's not that much of a shift in appetite or how they are satisfied.
Have you tried a log of calories eaten and calories burned? I do that; I did it for a successful 50-pound loss in 1994, and am doing it again now after regaining it all between 1996-2002.
I've noticed that really trying to log every bite not only helps me reduce intake, but initially, at least, researching the amount of calories in many of the foods I ate, that I thought were low-cal and healthy, was a real eye-opener. I'm not saying you don't know what you're eating, please don't get me wrong - and perhaps you've tried this already - but if I am for a few days slack in keeping my log, I immediately begin to stop losing. Silly, to be obsessed, but it works for me.
I keep a log of sleep data, too - as I don't have a card reader and already have a notebook of Obsessive Notes About My Body.....

I believe you, and while I can only speculate as to why it happens, I just wanted to let you know that I don't think it's all in your head. I've seen enough people mention weight gain (and have experienced it myself) that I'm convinced some people do gain weight because of something that happens when they go on the CPAP. Good luck.

I would have to say that without a doubt I have gained weight due to increased caloric intake, in other words, I'm eating a lot more. I do this because I'm always hungry now. I almost feel as though my stomach has stretched due to the CPAP machine keeping my stomach filled with air during the night. It takes more to fill me up and I keep eating throughout the day. I never used to do this. I'm actually struggling not to eat more. I eat so much now that I have heartburn and have sometimes thrown up since my stomach has grown so large that during my daily activities I feel sick to my stomach yet still hungry.

I am not a doctor but the first things that occur to me are possible GERD based on what you told me about the gas issues. Sometimes to get rid of the gas issues, people find that they have to turn down their CPAP pressure for a while until they adjust and then slowly work up to the titrated pressure. Have you started on any new medications since the weight started piling back on? Also, as we age, our metabolism naturally slows down which makes it easier to gain weight and harder to take it back off. I know when one of my friends started Paxil, he could not stop eating, constantly hungry, and so on and as soon as he started on a different medication, that desire went away. Maybe talk to a pharmacist about all your medications and see if any of them are likely to cause increased appetite. Just my best suggestions. I wish you good luck and I hope that things resolve for your soon and you find the answers you are searching for.
Blessings,
Maria

In addition to (watching) the calories.......it's the carbohydrates that will put the pounds on. And, if you have diabetes, that's what needs to be severely cut back.

If you slept better, you might be having pressure induced central apnea events. From what I've read here, apneas mess up your hormones and ability to lose weight. With the appetite, I'd still speak to a doctor about potentially being on the path for T2 diabetes.

Yes I did start taking meds when I started with CPAP due to high blood pressure. I didn't read about any side effects relating to increased appetite. You know, I don't remember ever feeling my stomach filled with air when I first started with this machine, but then again, I started with a setting of 8 and now I'm over 13. I'd like to try for a higher setting since I seem to be getting a better night sleep the higher I go, but I find it uncomfortable when trying to fall asleep and there is still that bloated feeling from this high pressure. I believe my worse craving is just before I go to sleep which I know is the worst time to munch. I understand about the suggestions re seeing a doctor but my wife just told me that her last visit to our GP had her waiting for 70 minutes and him walking out before she could talk about her second concern. Yes, in Ontario, we're short GP's.
Thanks for everyone’s suggestions; by the way, I just put on another pound.

I too gained weight once I started CPAP. I attributed it to the fact that I was actually going out more, with my then fiance, to dinner, parties, etc. Prior to CPAP, I would sleep a lot - take long naps daily and sleep long nights with only a few hours inbetween the nap and sleep and often skip dinner. Having more energy and going out and hitting the restauraunt scene was my downfall!

I’m also gaining weight after starting cpap and think you are correct. I believe the air in my stomach is either stretching it or making it feel empty
I wake up hungry and before cpap never was.
Can’t wait to eat when I get up and often want more soon after a meal that should keep me full for hours
When I first started cpap I would wake up with so much air I looked 9 months pregnant and it was terrible. Doc lowered the pressure but I’m still getting air in my stomach.
Going to ask about dropping pressure again but believe you are right about air in stomach leading to weight gain

Yes, still does after 11 years, being dormant for 11 years still true...
A body healing tends to operate better, requiring less fuel, however as you feel better, you will need to control your intake of food or move more to burn the CAL's, or the weight can creep up. Also you tend to get older every day, making it harder to burn fuel at the same levels you use to. Garbage in Garbage out, must be true!

Dowskidad; the reason you had to go back ELEVEN YEARS
to support your contention is evidence of some sort.
Remember that much crap gets published, simply because
publishing is given undue weight in evaluating a professional.
Just take one look at the number of "weight loss books".
Anybody can write--but writing bullshit is too damn easy.
