apnea and atrial fib

My sleep doctor, as well as my cardiologist, say that my atrial fibrillation (and flutter) might be related to apnea. I started cpap therapy about a month ago, but am finding it difficult to adjust to. So far, the therapy does not seem to help my heartbeat. Has anyone been successful in controlling his or her afib by cpap therapy?

OSA treatment hasn't helped my A fib. I can't help but think it is helping in some indirect way i.e., lower BP at night and better oxygen saturation.

I've been through verapamil and am currently using amiodarone (avoid if you can side affects get me, especially lack of UV protection) with a pacemaker. Pacemaker works like a charm. It stops the episodes from progressing, but I still know when the atrium isn't behaving.

I agree with your doctors, CPAP is the best thing you can do for your heart. While it took you years to develop AF, it can take some time to reverse that process. Did they also tell you the right side of your heart was enlarged? Ask your Cardiologist, I bet it is.

Overwork the heart from lack of oxygen or hypoxia and the right side will become larger it is after all a muscle. Only problem is when it becomes larger from being overworked or lack of oxygen in the blood only the right side gets the workout where it becomes larger than the left side, this throws off the normal beating rhythm of the heart where it no longer pumps as efficiently as it once did. So it is going to take some time for that heart muscle to relax and get back in sync with the other side. Using CPAP can take the load off the heart during sleep especially when it prevents the apnea that causes the heart to race to begin with.

BACKGROUND: We tested the hypothesis that patients with untreated obstructive sleep apnea (OSA) would be at increased risk for recurrence of atrial fibrillation (AF) after cardioversion. METHODS AND RESULTS: We prospectively obtained data on history, echocardiogram, ECG, body mass index, hypertension, diabetes, NYHA functional class, ejection fraction, left atrial appendage velocity, and medications in patients with AF/atrial flutter referred for DC cardioversion. Forty-three individuals were identified as having OSA on the basis of a previous sleep study. Data regarding the use of continuous positive airway pressure (CPAP) and recurrence of AF were obtained for 39 of these patients. Follow-up data were also obtained in 79 randomly selected postcardioversion patients (controls) who did not have any previous sleep study. Twenty-seven of the 39 OSA patients either were not receiving any CPAP therapy (n=25) or were using CPAP inappropriately (n=2). Recurrence of AF at 12 months in these 27 patients was 82%, higher than the 42% recurrence in the treated OSA group (n=12, P=0.013) and the 53% recurrence (n=79, P=0.009) in the 79 control patients. Of the 25 OSA patients who had not been treated at all, the nocturnal fall in oxygen saturation was greater (P=0.034) in those who had recurrence of AF (n=20) than in those without recurrence (n=5). CONCLUSIONS: Patients with untreated OSA have a higher recurrence of AF after cardioversion than patients without a polysomnographic diagnosis of sleep apnea. Appropriate treatment with CPAP in OSA patients is associated with lower recurrence of AF.

Thanks for your feedback, RiverDave and Snoredog.
I've had afib out of control for about 3 years and have had 2 ablations. Now I'm on Sotalol and I'm in sinus rhythm 1/2 of the time, but I don't like it because it slows down my heart too much.
I started cpap but still could not sleep more than 2 hours at a time. I'm hoping I'll feel better when I can sleep through all night. How long did it take for you to get used to the cpap?

I adapted to xPAP within a week. The only problem I had was too low of a starting pressure in APAP mode. Snoredog and Wulfman fixed that, though. Give it time, it is a big adjustment to make.

At least you're in sinus rhythm some of the time. I forgot about sotalol. I wasn't a candidate for normal ablation - I have an IVC, so they couldn't snake the cables where they needed to go. They offered transthoracic ablation, but the docs here hadn't done that many. The pacer is working well, though - been in AF for about 12 seconds since it was installed 9 months ago.
