Where is the provider contact info stored? Is it in the machine or the SD card?

It's not visible on the machine, at least not on mine. Under "My Provider" on the screen it has the serial number, model and software. So maybe on the SD card?

The provider information is on the paper work that came with your machine.

Mine has a sticker on it for my DME and I also got a magnet and a folder with their info.
