Well, this is actually something very reasonable and rational. It's how you get real handy data for analysis.
To me, it's something a bit off-topic to ask people here to answer a survey, specially for commercial purpose, but I'm not sure if it's really a big problem. However, the forum's rules should be followed if it's a forbidden situation.
Finally, about using the google's survey tool or asking for an email, I don't think it's a problem at all. I personally don't treat my email like a secret phone number that I only give to some few people, because even SPAM is not a problem in these days with modern email apps, but - in personal emails - I respect when others treat their email accounts this way.
But, again, it's optional to give it or not. If you don't wanna give your email because the possibility of some SPAM really upsets you, so don't, but only asking for your email should not be a big deal. It's not like he's asking for your credit card password

Maybe this will be useful for some of you, if you don't know it, but you can have a temporary email account, with basically zero effort and for free. I personally use Guerrilla Mail a lot, but there are several other websites that provide this kind of service for free. I hope it helps

Here is how i personally see it.
It's not a big deal if it's someone who has been here a while and is apart of the community.
Other then that they should first seek permission from forum owners and even they i would like to see them offer product free at time of inquiry for honest feedback.
I understand where OP is coming from going to lots of forums for his product development. I just think he rushed things in the way he went about it.

Exactly! Thank you for clarifying for me. I'm just looking for data on people's experiences to further my research and design. I want to make sure that when the product is launched it's the best version of what I envisioned it to be. I haven't found it that's for dam sure and I've tried them all. I'm hell bent on either finding something that will work or I may just have to create the dam thing myself like I did with my socks! It won't be a sock liner though lol. I'm just so frustrated and tired of being tired every day.

What personal info? It's an email address my friend. They survey asks for general everyday information about your experiences when using a mask with a liner. As mentioned this really isn't a big deal at all.
