New machine and increased AHI

Hi all... I'm a bit frustrated. I've had a ResMed S9 for the past 5+ years and I have averaged a 0.11 AHI on that unit (OI: 0.06, HI: 0.03,CAI: 0.01) . I just switched to the ResMed AirSense 10 autoset, and and I am using the same pressure settings, but now my AHI has averaged 0.75 since I started using it (OI: 0.46, HI: 0.19, CAI: 0.10, RERA: 0.03). What should I be looking at to figure out why my numbers are changing?

I also used to use the Swift FX nasal pillows and now I am using the AirFit p10 nasal Pillows. I used to keep the Swift FX really tight on my face and I do notice that the AirFit is more loose. My average leak rate for the past 12 months on the S9 was 0.85 (0.01% above leak rate threshold) and 8.10 (0.61% above leak rate threshold) with the AisSense 10.

Any help would be appreciated.

You are worrying for nothing. Those are fantastic numbers. After 10 years, I would love to have numbers like that...my average is 1.5 - 2.5

I understand but that still is a jump from what I was seeing over a very long period.

One thing I did notice is the EPR was turned ON with the new unit and was OFF on my old unit. Not sure if that would cause that to happen or not though but I will adjust it and see.

Make sure Every setting is the same as S9. Have you tried a couple nights with old Swift FX nasal pillows?

Yes EPR can make a difference.

I thought about the mask, but now that I see EPR was on, I think I will run with it off for a few nights before I switch masks. I do like the new mask (as does my wife since she no longer feels like she is sticking her head out the window in a moving car when she is next to me). With EPR on, I seem to recall that it reduces your effective pressure to some degree.

okay, so I switched off the EPR last night and my AHI was down to 0.5. A bit of an improvement, but not where I used to be still. I will give it a few more nights before I switch back to the Swift FX mask and see if that makes any difference.

I will say though that it felt better (more normal I guess) last night with the EPR off.

An ahi of 0.5 is really good figures that’s 4 events over an 8 hour period of sleep. Don’t fret about chasing anything lower..

To the OP. Do you really feel worse than before? I think that you're new machine might just be more sensitive than the older one.

It very well could be and that is something I have not considered. I guess the fact that I was so low in my AHI for almost 12 months prior and now it is around .5 (I know, still low) just bugs me.

So I have slept the last two night with an AHI of .2 and .1 respectively with the EPR turned off. The EPR being turned on must have been what was causing my higher than normal AHI. Glad I am back to what I normally see.
