Well...they can't put you in jail.
About all they can do is slap your hands and say "you shouldn't mess with things because you are too stupid to know what you are doing and we know everything because we walk on water"
It's a loaner machine anyway...they can't do a damn thing to you.
Wanna bet they end up wanting to give you the half assed brick ResMed AirSense CPAP because it costs them the least amount of money and your insurance will pay X amount no matter which model?

I am going rogue
I went rogue a couple of weeks ago. Was told to "not touch anything on the (loaner) machine except the on/off button". I started messing around with my pressure based on things I've picked up in this forum and am definitely sleeping better now! For sure do what you need to in order to optimize your therapy.

I feel like I might get in trouble from my provider!
Tell them to go to hell. If you don't talk like that, PM their phone number to me, and I'll tell them.

Don't let them talk you into a fixed pressure machine...You want an autoset.
Definitely ResMed AirSense AutoSet. Don't accept less!

Good morning, everyone.

Last night, I set my machine back to auto with a range of 5-15. That is what the machine defaulted to, and what my provider had it set to during my first week of treatment.

I woke up this morning, not feeling like I had a terrible night, or a great one. Checked my data, and certainly the AHI was way down last night from the nights before. To my untrained eye, it looks like a couple times my pressure was above 12, and one time up to the max 15?

I appreciate any interpretation from everyone here. Thank goodness for this forum.

You very briefly kissed 15 cm....probably again either REM related or on your back. It did happen during a time frame where it is very probable that REM could be happening.
Once you went to sleep the machine increased the pressure minimum to around 9.5 and never really dropped back down to the 5.
If it were me and I saw this often and I was going to use apap mode all the time I would probably increase the minimum from 5 to up around 8.5 or 9 and see what happens. The machine is going to go there and pretty much stay there anyway.

We have no control over REM related stuff. If there is a possibility pressure changes are related to REM a person might as well let the machine sort it out.

Supine sleeping we may have some control over but in my mind from personal experience, staying off one's back is easier said than done and I prefer to just sleep in any position I want and let the machine sort it out.

My own OSA is 5 times worse in REM than non REM and sometimes I need 6 to 8 cm more pressure in REM.
I have done some experiments and supine sleeping wasn't the factor...REM sleep was and confirmed by the sleep study done in a lab.

I think you can see what your options are. It's pretty easy to see and I think you have a good grasp on what is going on.
For me the why isn't so important as "how can I fix it".

Thank you. I find it frustrating that my provider just says, "There you go - you are good." without really looking at what is hapoening, or tweaking with the settings for the best result. If I had wanted no support or service, I would be buying online (and paying way less!). This forum is many times more helpful than my provider.

I agree with you. I am not too concerned with why things are happening - I just want to have good AHI and a nice sleep.

I put the machine back to cpap and will see if the AHI are back up again tomorrow. And the end of it all, I will be purchasing a machine and with the help of this group I am confident I can get my settings just right.
